基于“太少两感”理论论治大偻管窥

A Glimpse into Treating Major Rigidity Based on the "Greater Yang and Lesser Yin Co-Affection" Theory

摘要大偻之疾源于“阳气失于调和,寒气趁虚而入”,其核心病机在于肾督之气的衰弱与六淫邪气的侵袭,以肾督虚损为本,风寒湿邪等外邪为标。“太少两感”证即少阴阳虚兼太阳外感之证,其核心病机为少阴阳气不足。二者病机高度相似,且大偻病位在腰背部,与足太阳经在人体的循行路径相吻合,故可将大偻归属于“太少两感”证范畴。张仲景创制的麻黄附子细辛汤温经通阳,散寒除痹,能够温散表里两经之寒,开太阳之气机,扶少阴之阳气,促使侵入少阴的寒邪向外排出,治疗“太少两感”证之法,亦可用于大偻的治疗中。 Major rigidity arises from"disordered yang qi allowing cold pathogens to invade."Its core pathogenesis lies in the deficiency of kidney-governor vessel qi and invasion of exogenous pathogens,with intrinsic deficiency of the kidney-governor vessel as the root,and external wind-cold-dampness pathogens as the branch manifestations."Greater yang and lesser yin co-affection",characterized by yang deficiency of lesser yin combined with exterior syndrome of greater yang,shares a highly similar pathological mechanism with major rigidity.The disease location of major rigidity is in the lower back and spine,which aligns with the trajectory of the greater yang foot meridian in the body.Thus,major rigidity can be categorized under"greater yang and lesser yin co-affection"syndrome.Zhang Zhongjing′s Ephedra and Aconite and Asarum Decoction warms meridians,activates yang,disperses cold,and alleviates impediment.It can dispel cold from both meridians,unblock greater yang qi,reinforce lesser yin yang qi,and expel cold pathogens invading lesser yin through the greater yang exterior layer.This formula treats"greater yang and lesser yin co-affection"and can also be applied in removing major rigidity.
