早舒与浅镇静理念支持下的阶段性呼吸功能康复护理在重症肺炎机械通气患者中的应用

摘要 目的研究早舒及浅镇静理念支持下的阶段性呼吸功能康复护理在重症肺炎患者机械通气治疗中的应用效果。方法收集2024年1—8月150例接受机械通气治疗的重症肺炎患者临床病理资料,根据护理方案分为两组,每组75例。对照组接受常规护理,研究组接受早舒与浅镇静理念支持下的阶段性呼吸功能康复护理。记录并对比两组机械通气、自主呼吸恢复及ICU停留时间、呼吸、血流动力学指标、肺功能及不良反应。结果研究组机械通气、自主呼吸恢复及ICU停留时间短于对照组(P<0.05)。护理前,组间呼吸、血流动力学指标、肺功能各项指标差异无统计学意义(P>0.05)。护理后,组间呼吸频率(RR)、平均动脉压(MAP)差异无统计学意义(P>0.05);研究组氧合指数(PaO_(2)/FiO_(2))、心率(HR)高于对照组,动脉血二氧化碳分压(PaCO_(2))低于对照组(P<0.05);用力肺活量(FVC)、第1秒用力呼气容积(FEV 1)以及呼气峰流速(PEF)研究组高于对照组(P<0.05)。研究组不良反应发生率低于对照组(P<0.05)。结论早舒及浅镇静理念支持下的阶段性呼吸功能康复护理可显著改善重症肺炎机械通气患者机械通气时间、ICU停留时间及不良事件发生概率,且在应用较小镇静药剂量前提下,有利于改善患者肺功能、血流动力学指标。 Objective To study the application effect of phased respiratory function rehabilitation nursing supported by the concept of early relaxation and shallow sedation in mechanical ventilation treatment of patients with severe pneumonia.Methods The clinicopathological data of 150 patients with severe pneumonia who received mechanical ventilation from January to August 2024 were collected.The patients were divided into two groups of 75 cases each based on nursing protocols.The control group received routine nursing care,while the study group received phased respiratory function rehabilitation nursing supported by the concepts of early activity and light sedation.Mechanical ventilation,recovery of spontaneous respiration and ICU stay time,respiratory,hemodynamic indexes,lung function and adverse reactions were recorded and compared between the two groups.Results Mechanical ventilation,recovery of spontaneous respiration and ICU stay in the study group were shorter than those in the control group(P<0.05).Before nursing,there were no differences in respiratory,hemodynamic and pulmonary function indexes between groups(P>0.05).After nursing,there was no difference in respiratory rate(RR)and mean arterial pressure(MAP)between groups(P>0.05).The oxygenation index(PaO_(2)/FiO_(2))and heart rate(HR)in the study group were higher than the control group,and the partial pressure of CO_(2)(PaCO_(2))was lower than the control group(P<0.05).The forced vital capacity(FVC),forced expiratory volume for 1 second(FEV 1)and peak expiratory flow(PEF)in the study group were higher than the control group(P<0.05).The incidence of adverse reactions in the study group was significantly lower than that in the control group(P<0.05).Conclusion The staged respiratory function rehabilitation nursing supported by the concepts of early relaxation and shallow sedation can significantly improve the mechanical ventilation time,ICU stay time and the probability of adverse events in patients with mechanical ventilation of severe pneumonia,and is beneficial to improve the lung function and hemodynamic indexes of patients under the premise of using a smaller dose of sedative drugs.
