摘要
目的探讨1例真性红细胞增多症献血者参与无偿献血的发现过程及纠正与预防措施。方法按照《全血及成分血质量要求》中悬浮红细胞质量指标,以及《全血及成分血质量监测指南》规定的检测方法,对悬浮红细胞进行常规质量指标检测。结果质量抽检过程中发现,该献血者血液制备的悬浮红细胞中白细胞和血小板数量异常升高。通过回访了解到该献血者患有真性红细胞增多症,不符合献血者健康征询相关标准。经与献血者沟通,其捐献的血液按保密性弃血原则进行报废处理,并对献血者采取永久屏蔽措施。结论血站应加强一线采血医护人员的专业知识培训;强化血液制备、检测、质量抽检环节的管理;细化献血前检测项目检测标准,及时识别、发现不宜献血人群和不合格血液,杜绝不合格血液发往临床,确保受血者用血安全。
Objective To explore the identification process and corrective preventive measures for a case of polycythemia vera(PV)blood donor participating in voluntary blood donation.Methods Routine quality indicators of suspended red blood cells(SRBCs)were tested in accordance with the Quality Requirements for Whole Blood and Blood Components and the Guidelines for Quality Monitoring of Whole Blood and Blood Components.Results During quality sampling,abnormally elevated leukocyte and platelet counts were detected in the SRBCs prepared from the donor's blood.Follow-up revealed that the donor had polycythemia vera,which did not meet the health inquiry standards for blood donation.After communication with the donor,the donated blood was confidentially discarded,and the donor was permanently deferred.Conclusion Blood stations should enhance professional training for frontline medical staff,strengthen management of blood preparation,testing,and quality sampling processes,refine pre-donation testing criteria,and promptly identify ineligible donors or non-compliant blood products to prevent their clinical use and ensure transfusion safety.
