摘要
目的分析异基因造血干细胞移植(allo-HSCT)后出血性膀胱炎(HC)的危险因素。方法本研究为病例对照研究,选取2021年12月至2024年12月于北部战区总医院血液科行allo-HSCT并植活的47例血液病患者,男30例,女17例,年龄25(20,42)岁,年龄范围为15~53岁。收集并比较患者的临床资料,采用二元logistic回归分析筛选allo-HSCT后HC的危险因素。结果移植后发生HC 21例(Ⅰ级8例,Ⅱ级10例,Ⅲ级2例,Ⅳ级1例;早发型HC 2例,迟发型HC 19例),未发生HC 26例。发生HC与未发生HC患者的性别、供者来源、巨细胞病毒DNA定量、血小板植入时间比较,差异有统计学意义(P<0.05)。二元logistic回归分析结果表明,供者来源为父母是发生HC的独立危险因素(OR=6.044,95%CI:1.666~21.933,P=0.006)。结论供者来源为父母是发生HC的独立危险因素。
Objective To analyze risk factors for hemorrhagic cystitis(HC)after allogeneic hematopoietic stem cell transplantation(allo-HSCT).Methods This study was a case-control study,a total of 47 patients with hematological diseases who underwent allo-HSCT and survived transplantation in the department of Hematology at the General Hospital of the Northern Theater Command from December 2021 to December 2024 were selected,including 30 males and 17 females,with an age of 25.0(20.0,42.0)years old,ranging from 15 to 53 years old.The clinical data of the patients were collected and compared,and binary logistic regression analysis was used to screen the risk factors of HC after allo-HSCT.Results HC occurred in 21 cases after transplantation(gradeⅠ8 cases,gradeⅡ10 cases,gradeⅢ2 cases,gradeⅣ1 case;there were 2 cases of early-onset HC and 19 cases of late-onset HC),and 26 cases of non-occurrence HC.Comparison of gender,donor origin,quantification of cytomegalovirus DNA,and platelet implantation time between patients with HC and those without HC,the differences were statistically significant(P<0.05).The results of binary logistic regression analysis indicated that the source of donors being parents was an independent risk factor for HC(OR=6.044,95%CI:1.666 to 21.933,P=0.006).Conclusions The source of donors being parents was an independent risk factor for HC.
