下肢机器人训练联合神经肌肉电刺激对脑卒中后偏瘫患者步态及平衡功能的影响

The effects of lower limb robotic training combined with neuromuscular electrical stimulation on gait and balance function in post-stroke hemiplegic patients

摘要 目的分析下肢机器人训练联合神经肌肉电刺激对脑卒中后偏瘫患者步态及平衡功能的影响。方法选取86例脑卒中后偏瘫患者为研究对象,通过随机数字表法将患者分为实验组和对照组,每组43例。两组患者均接受常规康复治疗,实验组在常规康复治疗的基础上接受下肢机器人训练联合神经肌肉电刺激。比较两组患者治疗前及治疗4周后的步态时空参数(步频、步幅、步速、步行周期)、下肢运动功能[Fugl-Meyer运动功能评定量表(FMA)评分]步行功能[Holden步行功能分级(FAC)]、日常生活能力[改良Barthel指数评定量表(MBI)评分]及平衡功能[Berg平衡量表(BBS)评分]。结果治疗4周后,两组患者步行周期较治疗前缩短,且实验组步行周期(1.61±0.12)s短于对照组的(1.99±0.15)s,步频、步幅、步速较治疗前增大,且实验组步频(55.02±4.27)次/min、步幅(49.02±5.11)cm、步速(48.49±5.14)cm/s大于对照组的(44.51±6.24)次/min、(42.86±4.01)cm、(41.47±4.06)cm/s,差异有显著性意义(P<0.05)。治疗4周后,两组患者FMA评分、FAC较治疗前升高,且实验组FMA评分(26.63±2.33)分、FAC(3.86±0.74)级高于对照组的(21.42±2.13)分、(2.86±0.80)级,差异有显著性意义(P<0.05)。治疗4周后,两组患者MBI、BBS评分较治疗前升高,且实验组MBI评分(70.23±6.98)分、BBS评分(43.53±4.75)分高于对照组的(56.74±5.55)、(36.40±4.46)分,差异有显著性意义(P<0.05)。结论下肢机器人训练联合神经肌肉电刺激能够有效改善脑卒中后偏瘫患者的步态及平衡功能,提高患者生活质量。 Objective To analyze the effects of lower limb robotic training combined with neuromuscular electrical stimulation on gait and balance function in post-stroke hemiplegic patients.Methods A total of 86 cases of post-stroke hemiplegic patients were selected as study subjects.Patients were divided into an experimental group and a control group using a random number table according to random numerical table,with 43 patients in each group.Both groups received conventional rehabilitation therapy,while the experimental group additionally underwent lower limb robotic training combined with neuromuscular electrical stimulation.Patients in both groups were compared in terms of gait spatiotemporal parameters(step frequency,stride length,gait speed,gait cycle),lower extremity motor function[Fugl-Meyer assessment(FMA)score],walking function[functional ambulation category scale(FAC)],and activities of daily living[modified Barthel index(MBI)score]and balance function[Berg balance scale(BBS)score]before treatment and 4 weeks after treatment.Results After 4 weeks of treatment,the gait cycles in both groups of patients were shortened compared with those before treatment;moreover,the gait cycle(1.61±0.12)s in the experimental group was shorter than(1.99±0.15)s in the control group.The step frequency,stride length and gait speed were increased compared with those before treatment;the step frequency in the experimental group was(55.02±4.27)times/min,the step length was(49.02±5.11)cm,and the gait speed was(48.49±5.14)cm/s,which were greater than those in the control group[(44.51±6.24)times/min,(42.86±4.01)cm,and(41.47±4.06)cm/s];the difference was significantly significant(P<0.05).After 4 weeks of treatment,FMA scores and FAC in both groups of patients increased compared with those before treatment;the experimental group had FMA score of(26.63±2.33)points and FAC of(3.86±0.74)grades,which were higher than those in the control group[(21.42±2.13)points and(2.86±0.80)grades];the difference was significantly significant(P<0.05).After 4 weeks of treatment,MBI and BBS scores in both two groups of patients increased compared with those before treatment;the experimental group had MBI score of(70.23±6.98)points and BBS score of(43.53±4.75)points,which were higher than those in the control group[(56.74±5.55)and(36.40±4.46)points];the difference was significantly significant(P<0.05).Conclusion Lower limb robotic training combined with neuromuscular electrical stimulation effectively improves gait and balance function in post-stroke hemiplegic patients,thereby enhancing their quality of life.
