摘要
目的探讨血清生长分化因子-15(GDF-15)和趋化因子配体20(CCL20)在前列腺癌患者中的表达水平及其与临床病理特征的关系。方法选取2022年1月至2024年9月河北北方学院附属第二医院收治的前列腺癌患者128例。根据Gleason评分分组,≤6分为低危组(70例),7~10分为高危组(58例)。比较两组血清GDF-15、CCL20水平差异及肌层浸润发生率;采用Pearson相关分析高危组两指标相关性;通过Logistic回归分析高危前列腺癌的独立危险因素;利用ROC曲线评估两指标单独及联合对高危前列腺癌的诊断效能。结果高危组肌层浸润发生率高于低危组(P<0.05);高危组血清GDF-15和CCL20水平均高于低危组(均P<0.05);高危组内GDF-15与CCL20水平呈正相关(r=0.601,P<0.001);Logistic回归显示,肌层浸润、高GDF-15及高CCL20水平均为高危前列腺癌的独立危险因素(均P<0.05);ROC分析,血清GDF-15、CCL20单独及联合诊断高危前列腺癌的AUC值分别为0.837、0.741和0.892;二者联合诊断效能优于单独指标(均P<0.05)。结论GDF-15与CCL20水平在高危前列腺癌患者中升高,二者联合检测可提高高危前列腺癌的诊断效能,具有潜在临床价值。
Objective To clarify the levels and clinical significance of serum growth differentiation factor-15(GDF-15)and chemokine(C—C motif)ligand 20(CCL20)in prostate cancer patients.Methods From January 2022 to September 2024,128 prostate cancer patients accepted by the Second Affiliated Hospital of Heibei North University were included as the research subjects.Complying with the different Gleason scores,scores of 6 or below were considered low-risk,while scores between 7 and 10 were considered high-risk.Therefore,all research subjects were assigned into high-risk group(58 cases)and low-risk group(70 cases).Compare differences in serum GDF-15 and CCL20 levels and the proportion of muscle infiltration between the two groups.Pearson method was used to analyze the correlation between serum GDF-15 and CCL20 in high-risk prostate cancer patients.Logistic method was used to analyze the related factors of high-risk prostate cancer.ROC curve was used to analyze the efficacy of serum GDF-15 and CCL20 in predicting high-risk prostate cancer.Results The proportion of muscle infiltration in the high-risk group was prominently higher than that in the low-risk group(P<0.05).The serum GDF-15 and CCL20 in the high-risk group were prominently higher than those in the low-risk group(all P<0.05).The serum GDF-15 and CCL20 in high-risk prostate cancer patients were positively correlated(r=0.601,P<0.001).Logistic method showed that muscle infiltration,and high serum GDF-15 and CCL20 were independent risk factors for high-risk prostate cancer(all P<0.05).The area under the ROC curve(AUC)of serum GDF-15 and CCL20,and their combination in predicting high-risk prostate cancer was 0.837,0.741,and 0.892,respectively.The combined prediction of the two was superior to the individual diagnoses(all P<0.05).Conclusions Serum GDF-15 and CCL20 are upregulated in high-risk prostate cancer patients,combined detection improves the diagonstic performance for high-risk prostate cancer and has potential clinical value.
