摘要
神经重症患者早期,联合诊治可降低其病死率和致残率、缩短住院时间,对加快恢复进程具有重要意义。通过“中枢-外周-中枢”闭环理论指导,试将中医学与现代康复疗法融合,探讨神经重症针康防治体系的构建思路。应用头针、高压氧、电刺激等中枢干预,结合体针、体位转换、多感官刺激等外周干预以促进意识状态恢复;以头针、运动想象、视觉反馈训练等中枢干预联合体针、运动疗法、功能性电刺激、中医导引、推拿治疗等外周干预是防治重症监护病房(ICU)获得性肌无力的重要策略;中枢干预中头针、重复经颅磁刺激、经颅直流电刺激联动体针、中医情志疗法、传统功法等外周干预手段,可调畅情志,改善精神、心理问题及认知功能障碍。“中枢-外周-中枢”闭环理论通过协同调控大脑与肢体,实现正反馈的双向提升,代表了现代康复医学领域未来发展的趋势。在此理论的指导下,构建的神经重症针康防治体系不仅突出了中医药的优势特色,而且体现了继承与创新的结合,具有前瞻性和临床指导意义。
Early combined diagnosis and treatment can significantly reduce the mortality and disability rates of neurocritical disorders patients,shorten hospital stays and accelerate the recovery process.Guided by the“central-peripheralcentral”closed-loop theory,this study aims to integrate traditional Chinese medicine with modern rehabilitation therapies to explore the construction of an acupuncture-rehabilitation prevention and treatment system for neurocritical disorders.Central interventions,such as scalp acupuncture,hyperbaric oxygen therapy and electrical stimulation,combined with peripheral interventions,including body acupuncture,position changes,and multisensory stimulation,are applied to promote the restoration of consciousness.For the prevention and treatment of intensive care unit(ICU)-acquired weakness,a combination of central interventions(scalp acupuncture,motor imagery and visual feedback training)and peripheral interventions(body acupuncture,exercise therapy,functional electrical stimulation,traditional Chinese medical guidance and tuina therapy)serves as a crucial strategy.Additionally,central interventions involving scalp acupuncture,repetitive transcranial magnetic stimulation and transcranial direct current stimulation,in conjunction with peripheral interventions like body acupuncture,traditional Chinese emotional therapy,and traditional exercises,can regulate emotions and improve mental,psychological issues and cognitive impairment.The“central-peripheral-central”closed-loop theory achieves bidirectional positive feedback enhancement by synergistically regulating the brain and limbs,representing a future trend in modern rehabilitation medicine.Under the guidance of this theory,the constructed acupuncture-rehabilitation prevention and treatment system for neurocritical disorders not only highlights the advantages and characteristics of traditional Chinese medicine but also embodies the integration of inheritance and innovation,demonstrating forward-looking and clinical guiding significance.
