摘要
目的 探讨股骨椭圆隧道技术对膝关节前交叉韧带重建术后步态的影响。方法 选取2022年6月至2024年7月收治的膝关节前交叉韧带重建术患者115例。按随机数字表法分成对照组和观察组,对照组术中采取圆隧道技术治疗,观察组予以股骨椭圆隧道技术治疗。记录患侧足底各区冲量及足偏角;记录双侧腿肌力,包括膝关节屈/伸肌峰力矩(PT)、总功(TW)、平均功率(AP)和屈/伸肌单次最大做功量(MRTW);采用Lysholm膝关节功能评分评估膝关节功能。结果 两组术前足偏角、Lysholm膝关节功能评分无显著差异(P>0.05);术后观察组患侧足偏角显著小于对照组,Lysholm膝关节功能评分显著高于对照组(P<0.05)。两组术前前足区、中足区冲量百分比无显著差异(P>0.05),术后观察组患侧足底各区冲量显著优于对照组(P<0.05)。两组术前PT、TW、AP、MRTW差异无统计学意义(P>0.05);术后两组PT、TW、AP、MRTW均显著改善(P<0.05),观察组术后PT、TW、AP、MRTW显著优于对照组(P<0.05)。结论 股骨椭圆隧道技术可改善膝关节前交叉韧带重建术后患侧足底各区冲量、PT、TW及足偏角,加速膝关节恢复。
Objective This study aimed to explore the effect of the femoral elliptical tunnel technique on gait after anterior cruciate ligament reconstruction of the knee joint.Methods A total of 115 patients undergoing anterior cruciate ligament reconstruction of the knee joint between June 2022 and July 2024 were selected.The patients were divided into a control group and an observation group according to the random number table method.The control group was treated with the circular tunnel technique during the surgery,while the observation group was treated with the femoral elliptical tunnel technique.The impulse and foot angle deviation of each area on the affected foot sole were recorded.The muscle strength of both legs,including the peak moment(PT),total work(TW),average power(AP)of the flexor/extensor muscles of the knee joint,and the maximum single work done by the flexor/extensor muscles(MRTW)was recorded.Knee joint function was evaluated using the Lysholm knee joint function score.Results No significant differences were found in the preoperative deviations in the foot angles and Lysholm knee joint function scores between the two groups(P>0.05).After the surgery,the deviations in the angle of the affected foot in the observation group were significantly smaller than in the control group,and the Lysholm knee joint function score was significantly higher than in the control group(P<0.05).No significant difference was observed in the percentage of impulses in the forefoot and midfoot areas between the two groups before surgery(P>0.05).After surgery,the impulses in each area of the foot sole on the affected side were significantly improved in the observation group compared with the control group(P<0.05).There were no statistically significant differences in PT,TW,AP,and MRTW before surgery between the two groups(P>0.05).After surgery,PT,TW,AP,and MRTW in both groups were significantly improved(P<0.05),with PT,TW,AP,and MRTW in the observation group being significantly better than the control group(P<0.05).Conclusion The femoral elliptical tunnel technique can improve impulses,PT,TW,and foot angle deviations of each area on the affected foot after anterior cruciate ligament reconstruction of the knee joint,and accelerate recovery of the knee joint.
