精准营养疗法对肾衰竭血液透析患者机体营养状态及预后的影响

摘要目的观察精准营养疗法对改善慢性肾衰竭(CRF)血液透析患者机体营养状态及预后的影响。方法选取100例CRF患者为研究对象,按照随机数字表法分为常规组和精准组,每组50例。常规组实施常规营养管理,精准组实施精准营养管理,比较两组的营养状态、免疫功能、钙磷代谢情况及不良预后发生情况。结果在不同管理模式下,精准组的前白蛋白(PA)、血红蛋白(Hb)、血清白蛋白(ALB)、血清转铁蛋白(TRF)均高于常规组,差异有统计学意义(P<0.05)。精准组的T淋巴细胞亚群CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)均高于常规组,CD8^(+)低于常规组(P<0.05)。精准组的血钙水平高于常规组,血磷水平低于常规组(P<0.05)。精准组的不良预后总发生率4.00%(2/50)低于常规组20.00%(10/50)(P<0.05)。结论精准营养疗法能改善CRF血液透析患者的营养状态及免疫功能,对稳定机体钙磷代谢水平并降低不良预后发生风险均有积极意义。 Objective To observe the effect of precision nutritional therapy on the nutritional status and prognosis of hemodialysis patients with chronic renal failure(CRF).Methods 100 CRF patients were selected as the research subjects and randomly divided into a conventional group were and a precision group,with 50 cases in each group.The conventional group received standard nutritional management,while the precision group received precision nutritional management.The nutritional status,immune function,calcium and phosphorus metabolism,and adverse prognosis of the two groups of patients were compared.Results Under different management modes,the precision group had higher levels of albumin(PA),hemoglobin(Hb),serum albumin(ALB),and serum transferrin(TRF)than the conventional group,showing a statistically significant difference(P<0.05).The T lymphocyte subsets CD3^(+),CD4^(+),and CD4^(+)/CD8^(+)in the precision group were higher than those in the conventional group,while CD8^(+)was lower(P<0.05).The blood calcium level in the precision group was higher,and the blood phosphorus level was lower compared to the conventional group(P<0.05).The incidence of poor prognosis in the precision group was 4.00%(2/50)lower than that in the conventional group,which was 20.00%(10/50)(P<0.05).Conclusion Precision nutrition therapy can improve the nutritional status and immune function of CRF hemodialysis patients,and has a positive significance in stabilizing the body′s calcium and phosphorus metabolism levels and reducing the risk of adverse prognosis.
