摘要
目的:观察维药复方吉多果化石蜜膏联合体外冲击波碎石术(Extracorporeal Shock Wave Lithotripsy,ESWL)对输尿管结石患者术后炎症因子、肾功能指标、术后并发症和临床疗效的影响。方法:以接受诊疗的92例输尿管结石患者为研究对象,将其随机分为观察组与对照组,各46例。对照组应用ESWL治疗,术后予常规对症治疗。观察组在对照组基础上应用复方吉多果化石蜜膏治疗。疗程为2周。比较两组治疗前后的临床疗效、炎症因子、肾功能指标、术后并发症等。结果:观察组总有效率高于对照组,差异有统计学意义(P<0.05);观察组感染、肾绞痛、血尿等术后并发症发生率明显低于对照组(P<0.05);两组尿微量白蛋白(Microalbuminuria,mALB)、β_(2)微球蛋白(β_(2)–microglobulin,β_(2)–MG)、血肌酐水平均比治疗前下降,且观察组血清β_(2)–MG、血肌酐和尿mALB水平明显低于对照组(P<0.01);与治疗前相比,治疗后两组患者血清C反应蛋白(C–reactive Protein,CRP)、肿瘤坏死因子–α(Tumor Necrosis Factor–alpha,TNF–α)、白细胞介素–6(Interleukin–6,IL–6)水平下降,且观察组下降程度较对照组显著(P<0.05)。结论:在ESWL治疗的基础上联合应用复方吉多果化石蜜膏能有效提高对输尿管结石的疗效,减少术后并发症,降低炎性反应和肾功能损伤。
Objective:To observe the effect of Uyghur medicine the Fufang Jiduoguo Huashi Mi Gao(复方吉多果化石蜜膏)plus extracorporeal shock wave lithotripsy on postoperative ureteral calculi and its influences on inflammatory factors,renal function indexes,and postoperative complications of the patients.Methods:A total of 92 ureteral calculi patients were randomly divided into the observation group and the control group,with 46 cases in each group.The control group was treated by extracorporeal shock wave lithotripsy and routine symptomatic treatment after surgery,while the observation group was treated with the Fufang Jiduoguo Huashi Mi Gao on the basis of the treatment plan in the control group.The treatment course was two weeks.The clinical efficacy,inflammatory factors,renal function indexes,postoperative complications and other indicators between two groups before and after treatment were compared after the treatment.Results:The total efficiency in the observation group was higher than the control group,and the difference was statistically significant(P<0.05).The total incidence of postoperative complications such as infection,renal colic,and hematuria in the observation group was significantly lower than the control group(P<0.05).The levels of microalbuminuria(mALB),β_(2)-microglobulin(β_(2)-MG),and serum creatinine in both groups after treatment were decreased compared with before treatment,and the observation group showed lower levels of mALB,β_(2)-MG,and serum creatinine than the control group(P<0.01).Compared with before treatment,the levels of CRP,TNF-α,and IL-6 were decreased in both groups after treatment,and the decrease in the observation group was more obvious than the control group(P<0.05).Conclusion:The Fufang Jiduoguo Huashimi Mi Gao plus extracorporeal shock wave lithotripsy can effectively improve the efficacy on postoperative ureteral calculi,reduce postoperative complications,and lower inflammation reaction and renal function damage.
