摘要
急性脑梗死(ACI)作为临床常见且危急的脑血管疾病,其病理机制涉及脑组织急性缺血缺氧引发的级联损伤,具有高发病率、高致残率及高死亡率特征。中医将ACI归属于“中风”“卒中”范畴,结合现代临床研究,认为其病因病机主要涵盖风、火、痰、瘀、虚等要素,尤其以气虚血瘀证贯穿疾病发生发展全程。近年来,中医药在ACI治疗中展现独特优势,通过辨证论治,运用益气活血、化痰通络、平肝熄风等治法,配合经典方剂(如补阳还五汤、天麻钩藤饮等)口服,结合针灸、中医定向透药疗法、穴位贴敷等特色疗法,可有效改善患者神经功能缺损症状,调节血液流变学指标,减轻炎症反应,促进神经修复。系统梳理ACI中医病因病机研究进展,深入分析辨证论治方案及中西医结合治疗策略,旨在为中医药干预ACI的临床实践与机制研究提供参考。
Acute cerebral infarction(ACI),a common and critical cerebrovascular disease in clinical practice,involves a cascade of injuries caused by acute ischemia and hypoxia in brain tissue,characterized by high incidence,disability,and mortality rates.In traditional Chinese medicine(TCM),ACI is classified under the categories of"wind stroke"or"apoplexy".Combined with modern clinical research,its etiology and pathogenesis are attributed to factors such as wind,fire,phlegm,blood stasis,and deficien-cy,with qi deficiency and blood stasis being particularly prominent throughout the disease progression.In recent years,TCM has dem-onstrated unique advantages in treating ACI.Through syndrome differentiation and treatment,therapeutic methods such as replenishing qi and activating blood circulation,resolving phlegm and unblocking collaterals,and calming the liver to extinguish wind are employed.These are complemented by classical prescriptions(e.g.,Buyang Huanwu decoction,Tianma Gouteng decoction)for oral administra-tion,alongside specialized therapies like acupuncture,TCM directional drug penetration therapy,and acupoint application.These ap-proaches effectively improve neurological deficits,regulate hemorheological indices,reduce inflammatory responses,and promote neural repair.This paper systematically reviews the progress in TCM research on the etiology and pathogenesis of ACI,provides an in-depth analysis of syndrome differentiation and treatment strategies,and explores integrated TCM and Western medicine approaches,aiming to offer references for clinical practice and mechanistic research on TCM interventions for ACI.
