摘要
方法:通过计算机检索中国知网(CNKI)、万方数据知识服务平台、中国生物医学文献数据库(CBM)、维普中文期刊服务平台(VIP)、The Cochrane Library、PubMed、Embase、Web of Science等数据库中公开发表的有关经皮穴位电刺激治疗脑卒中患者手功能障碍的所有随机对照试验(RCT),检索时限为数据库建库以来至2024年7月的中英文文献,整个检索、筛查以及纳入研究的偏倚风险评价均由2位研究者独立进行,数据采用RevMan 5.3软件进行Meta分析。结果:最终纳入14篇随机对照试验,共计937名患者。Meta分析显示,试验组与对照组的改良Ashworth量表评分具有显著性差异[WMD=-0.38,95%CI(-0.40,-0.36),P<0.00001],且改良Barthel指数评分也具有显著性差异[WMD=10.51,95%CI(8.73,12.28),P<0.00001]。结论:通过数据处理分析,表明经皮穴位电刺激可以有效缓解脑卒中手功能障碍患者的手痉挛程度,提高患者的日常生活能力,促进脑卒中后手功能障碍的康复。但由于纳入文献研究方法学上整体质量不高,后续仍需更多大样本、多中心和高质量的研究验证。
Objective To evaluate the clinical effect of percutaneous acupoint electrical stimulation on the rehabilitation of hand function in stroke patients by meta-analysis.Methods CNKI,Wanfang Data Knowledge Service Platform,China Biomedical Litera-ture Database(CBM),VIP Chinese Journal Service Platform,The Cochrane Library,PubMed,Embase and Web of were searched by computer All randomized controlled trials(RCTS)on percutaneous acupoint electrical stimulation in the treatment of hand dysfunc-tion in stroke patients published in Science and other databases were searched for Chinese and English literature from the establishment of the database to July 2024,and the entire search,screening and bias risk assessment of included studies were carried out independ-ently by two investigators.Data were meta-analyzed using RevMan 5.3 software.Results Fourteen randomized controlled trials with a total of 937 patients were included.Meta-analysis showed that the modified Ashworth scale score of the experimental group and the control group had significant differences[WMD=-0.38,95%CI(-0.40,-0.36),P<0.00001],and the modified Barthel index score also had significant differences[WMD=10.51,95%CI(8.73,12.28),P<0.00001].Conclusion Through data processing and analysis,the results show that percutaneous acupoint electrical stimulation can effectively relieve the degree of hand spasm in stroke patients with hand dysfunction,improve the patients'daily living ability,and promote the rehabilitation of hand dys-function after stroke.However,due to the low methodological quality of the included literature,more large-sample,multi-center and high-quality research verification is still needed in the future.
