呼吸八段锦在慢性阻塞肺疾病稳定期患者中的应用及对血气指标和CAT、MCFS评分的影响

Application of Baduanjin in Patients with Stable COPD and Impact on Blood Gas Indicators,CAT Score and MCFS Score

摘要 目的观察呼吸八段锦对慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)稳定期患者血气指标、COPD评估测试(COPD assessment test,CAT)评分及曼彻斯特COPD疲劳量表(Manchester COPD Fatigue Scale,MCFS)评分的影响。方法选取该院2022年1月—2023年10月接受常规治疗的COPD稳定期患者96例为研究对象,采用随机信封法分为对照组和观察组,每组48例,对照组单纯行常规治疗,观察组在对照组基础上实施呼吸八段锦治疗。治疗前后分别采集两组患者血液,分别采用全自动血气分析仪、肺功能检测仪评估患者血气指标和肺功能指标,包括动脉氧分压(arterial partial pressure of oxygen,PaO_(2))、动脉二氧化碳分压(arterial partial pressure of carbon dioxide,PaCO_(2))、血氧饱和度(blood oxygen saturation,SaO_(2))和第1秒用力呼气量(forced expiratory volume in 1 second,FEV1)、第1秒用力呼气容积占预计值百分比(FEV1 to predicted value ratio,FEV1%pred)、用力肺活量(forced vital capacity,FVC)、呼气峰流量(peak expiratory flow,PEF);采用CAT评分、MCFS评分评估患者的综合征状和特异性疲劳度,最后进行COPD疗效评定。结果治疗2个月后,观察组血气指标包括PaO_(2)、PaCO_(2)、SaO_(2)水平分别为88.87 mm Hg、38.24 mm Hg、95.16%,肺功能指标FEV1、FEV1%pred、FVC、PEF水平分别为(3.79±0.75)L、(62.45±5.22)%、(3.15±0.53)L、(4.63±0.89)L/S,除PaCO_(2)低于对照组外其余指标均显著高于对照组,差异均有统计学意义(P<0.05);CAT评分、MCFS评分分别为23.81、19.45分,均显著低于对照组,差异有统计学意义(P<0.05);观察组疗效总有效率为87.50%(42/48),显著高于对照组,差异有统计学意义(P<0.05)。结论呼吸八段锦可有效促进COPD稳定期患者的健康恢复,降低患者特异性疲劳和功能障碍,同时能提高血气指标,改善呼吸功能,临床治疗效果显著。 Objective To observe the effects of Baduanjin on blood gas indicators,COPD assessment test(CAT)score and Manchester COPD Fatigue Scale(MCFS)score of patients with stable chronic obstructive pulmonary disease(COPD).Methods A total of 96 patients with stable COPD who received conventional treatment in the hospital from January 2022 to October 2023 were selected as the study subjects.They were randomly divided into a control group and an observation group by the envelope method,with 48 cases in each group.The control group received conventional treatment alone,and the observation group received Baduanjin on this basis.Blood blood gas indicators[arterial partial pressure of oxygen(PaO_(2)),arterial partial pressure of carbon dioxide(PaCO_(2))and blood oxygen saturation(SaO_(2))]and pulmonary function indicators[forced expiratory volume in 1 second(FEV1),FEV1 to predicted value ratio(FEV1%pred),forced vital capacity(FVC)and peak expiratory flow(PEF)]were detected before and after treatment.CAT and MCFS were used to evaluate comprehensive symptoms and specific fatigue,respectively.The therapeutic effect on COPD was evaluated.Results After 2 months of treatment,the values of PaO_(2),SaO_(2),FEV1,FEV1%pred,FVC and PEF in the observation group were significantly higher than those in the control group.The value of PaCO_(2)in the observation group was lower than that in the control group.The differences were statistically significant(P<0.05).CAT score and MCFS score of the observation group were significantly lower than those of the control group,with statistically significant difference(P<0.05).The total effective rate of the observation group was significantly higher than that of the control group,with a statistically significant difference(P<0.05).Conclusion Baduanjin can effectively promote the recovery of patients with stable COPD,reduce their specific fatigue and functional impairment,and improve blood gas indicators and respiratory function,with significant clinical effect.
