摘要
目的分析内镜、艾司奥美拉唑及生长抑素治疗高危上消化道溃疡出血的效果。方法方便选取2019年12月—2023年2月赣南医科大学附属萍乡医院的175例高危上消化道溃疡出血患者为研究对象。根据治疗方案分为常规组(n=85)与生长抑素组(n=90),常规组给予内镜联合艾司奥美拉唑治疗,生长抑素组在常规组的基础上给予生长抑素治疗。比较两组治疗有效率、再出血率、转手术率、住院时间、胃液pH值、Blatchford评分及不良反应发生情况。结果两组治疗有效率、再出血率、转手术率、住院时间比较,差异均无统计学意义(P均>0.05)。生长抑素组胃液pH值为6.67±0.38,高于常规组的5.53±0.38,生长抑素组Blatchford评分为(3.61±0.75)分,低于常规组的(5.58±0.77)分,差异均有统计学意义(t=19.631,17.026;P均<0.05)。生长抑素组心悸发生率、头晕发生率、头痛发生率均高于常规组,差异均有统计学意义(P均<0.05)。结论内镜治疗高危消化性溃疡出血患者后,单独使用艾司奥美拉唑和艾司奥美拉唑+生长抑素疗效相近,但使用生长抑素治疗可改善患者胃液pH值及Blatchford评分。
Objective To analyze the efficacy of endoscopy,esomeprazole,and somatostatin in the treatment of high-risk upper gastrointestinal ulcer bleeding.Methods A total of 175 patients with high-risk upper gastrointestinal ulcer bleeding admitted to Gannan Medical University Affiliated Pingxiang Hospital from December 2019 to February 2023 were conveniently selected as the research subjects.They were divided into a conventional group(n=85)and a somatostatin group(n=90)according to treatment protocols.The conventional group received endoscopy combined with esomeprazole,while the somatostatin group received somatostatin on the basis of the conventional treatment.The treatment efficiency,rebleeding rate,conversion rate to surgery,hospital stay,gastric juice pH value,Blatchford score,and incidence of adverse reactions were compared between the two groups.Results There were no statistically significant differences in treatment efficiency,rebleeding rate,conversion rate to surgery,or hospital stay between the two groups(all P>0.05).The gastric juice pH value in the somatostatin group was 6.67±0.38,significantly higher than 5.53±0.38 in the conventional group,and the Blatchford score in the somatostatin group was 3.61±0.75,lower than 5.58±0.77 in the conventional group,and the differences were statistically significant(t=19.631,17.026;all P<0.05).The incidences of palpitation,dizziness,and headache in the somatostatin group were higher than those in the conventional group,and the differences were statistically significant(all P<0.05).Conclusion After endoscopy for high-risk peptic ulcer bleeding,esomeprazole alone and esomeprazole combined with somatostatin show similar curative effects,but somatostatin treatment can improve gastric juice pH value and Blatchford score.
