摘要
目的探讨电针联合舒筋壮骨汤对经皮椎体后凸成形术(percutaneous kyphoplasty,PKP)后患者骨密度及腰椎功能的影响。方法采用回顾性研究方法,选取2022年1月至2023年11月期间在医院行PKP治疗的脊柱压缩性骨折患者76例,根据干预方法的不同分为两组。对照组(38例)术后给予舒筋壮骨汤,观察组(38例)在此基础上加用电针刺激。对比两组骨密度、血钙、血清β-胶原特殊序列(β-C-terminal cross-linking telopeptide of typeⅠcollagen,β-CTx)水平,腰椎功能[Oswestry功能障碍指数(Oswestry disability index,ODI)和日本骨科学会评分标准(Japanese Orthopaedic Association score,JOA)评分],骨质疏松症状分级。结果干预后3个月,两组的骨密度和血钙水平均较干预前提高,且观察组的提高幅度显著大于对照组(P<0.05);血清β-CTx水平降低,且观察组的降低幅度显著大于对照组(P<0.05)。干预后3个月,两组ODI评分较干预前下降,JOA评分较干预前提高,且观察组ODI评分低于对照组,JOA评分高于对照组,差异有统计学意义(P<0.05)。干预后3个月,两组各项症状评分较干预前下降,且观察组腰背疼痛、腰膝酸软、下肢痿弱、下肢疼痛、步履维艰评分均低于对照组,差异有统计学意义(P<0.05)。结论电针联合舒筋壮骨汤用于PKP术后患者的效果优于单用舒筋壮骨汤,能够改善患者骨密度,促进骨折愈合,有助于腰椎功能恢复。
Objective To explore the effect of electroacupuncture combined with Shujin Zhuanggu decoction on bone mineral density and lumbar function of patients after percutaneous kyphoplasty(PKP).Methods A retrospective study was conducted,and 76 patients with vertebral compression fractures treated with PKP from January 2022 to November 2023 in our hospital were selected and divided into two groups according to different intervention methods.The control group(38 cases)was given Shujin Zhuanggu decoction after operation,and the observation group(38 cases)was given electroacupuncture stimulation on the basis of this.Bone mineral density,blood calcium,and serumβ-C-terminal cross-linking telopeptide of typeⅠcollagen(β-CTx)levels,lumbar function[Oswestry disability index(ODI)and Japanese Orthopaedic Association score(JOA)]and osteoporosis symptom grading were compared between the two groups.Results Three months after the intervention,the bone mineral density and blood calcium levels of both groups were higher than those before the intervention,but the observation group was significantly higher than the control group.The serumβ-CTx level decreased,and the observation group was lower than the control group,with statistical significance(P<0.05).Three months after the intervention,the ODI score of the two groups decreased and the JOA score increased,and the ODI score of the observation group was lower than that of the control group,and the JOA score was higher than that of the control group,with statistical significance(P<0.05).Three months after the intervention,the scores of symptoms in the two groups decreased compared with those before the intervention,and the scores of low back pain,weakness of waist and knees,weakness of lower limbs,pain of lower limbs and difficulty in walking in the observation group were lower than those in the control group,with statistical significance(P<0.05).Conclusion The effect of electroacupuncture combined with Shujin Zhuanggu decoction in patients after PKP is better than that of Shujin Zhuanggu decoction alone,which can improve patients’bone density,promote fracture healing and contribute to the recovery of lumbar function.
