摘要
目的探讨双模态超声定量或半定量评估妊娠期糖尿病患者胎盘功能的价值。方法回顾性选取2020年1月至2023年12月南阳医学高等专科学校第一附属医院收治的90例妊娠期糖尿病患者作为研究对象,根据其病情严重程度分为轻度组(n=51)和中重度组(n=39),另选取54例同期血糖正常孕妇为对照组。采用常规超声和三维能量多普勒超声检查孕妇的胎盘功能,并分析双模态超声定量或半定量评估妊娠期糖尿病患者胎盘功能的临床价值。结果定量分析结果显示,子宫动脉PI、RI、S/D值:对照组<轻度组<中重度组,差异均具有统计学意义(P<0.05)。半定量分析结果显示,三维能量指数VI、FI、VFI值:对照组>轻度组>中重度组,差异均具有统计学意义(P<0.05)。ROC曲线显示,双模态超声定量、半定量联合评估妊娠期糖尿病患者胎盘功能的AUC为0.856,均高于各单一参数评估(P<0.05)。结论双模态超声可有效评估妊娠期糖尿病患者的胎盘功能,有利于临床尽早进行诊治监测。
Objective To explore the value of bimodal ultrasound in quantitative or semi-quantitative evaluation of placental function in patients with gestational diabetes mellitus.Methods 90 patients with gestational diabetes mellitus admitted to the First Affiliated Hospital of Nanyang Medical College from January 2020 to December 2023 were retrospectively selected as the research subjects and divided into the mild group(n=51)and the moderate-severe group(n=39)according to the disease severity.54 pregnant women with normal blood glucose during the same period were selected as the control group.The placental functions of pregnant women were detected by conventional ultrasound and three-dimensional energy Doppler ultrasound.The clinical value of bimodal ultrasound in quantitative or semi-quantitative evaluation of placental function in patients with gestational diabetes mellitus was analyzed.Results The results of quantitative analysis indicated that the values of PI,RI,and S/D of uterine artery followed the order:control group<mild group<moderate-severe group,and all these differences were statistically significant(P<0.05).The findings of semi-quantitative analysis indicated that the values of three-dimensional energy Doppler indicators,including VI,FI,and VFI,followed the order:control group>mild group>moderate-severe group,and all these differences were statistically significant(P<0.05).The ROC curve showed that the AUC of bimodal ultrasound for the combined quantitative and semi-quantitative assessment of placental function in patients with gestational diabetes mellitus was 0.856,higher than that of assessments based on each single parameter(P<0.05).Conclusions Bimodal ultrasound can effectively evaluate placental function in patients with gestational diabetes mellitus,facilitating early clinical diagnosis,treatment,and monitoring.
