摘要
探讨高黏度骨水泥(high-viscosity cement,HVC)经皮椎体成形术(percutaneous vertebro plasty,PVP)治疗骨质疏松性胸腰椎骨折(osteoporotic vertebral compression fracture,OVCF)患者的临床疗效。选取2022年1月—2024年1月医院收治的100例胸腰椎单节段OVCF患者为研究对象,随机将其分为HVC组(n=50)与常规黏度骨水泥(conventional-vis-cosity cement,CVC)组(n=50)。比较两组患者的围手术期指标(手术时间、骨水泥注入量)、影像学参数(渗漏率、骨水泥分布)、疼痛视觉模拟评分(VAS)、Oswestry功能障碍指数(ODI)及并发症发生情况。结果显示,两组患者的手术时间、骨水泥注入量比较,无统计学差异(P>0.05);HVC组骨水泥渗漏率显著低于CVC组,骨水泥分布优良率高于CVC组(P<0.05);HVC组术后72 h的VAS评分改善效果更显著,术后3个月的ODI评分更低(P<0.05);两组患者的并发症发生率比较,无统计学差异(P>0.05)。研究发现,HVC可显著降低PVP术中骨水泥渗漏风险,短期疼痛缓解及功能恢复效果更优,是治疗OVCF的有效选择。
This study explores the clinical efficacy of high-viscosity cement(HVC)in percutaneous vertebroplasty(PVP)for osteoporotic vertebral compression fractures(OVCF).A total of 100 patients with single-segment OVCF of the thoracolumbar spine admitted to the hospital from January 2022 to January 2024 were randomly divided into an HVC group(n=50)and a conventional-viscosity cement(CVC)group(n=50).Perioperative indicators(operation time,bone cement injection volume),imaging parameters(leakage rate,bone cement distribution),visual analog scale(VAS)for pain,Oswestry disability index(ODI),and complications were compared between the two groups.Results showed no signifi cant differences in operation time or bone cement injection volume between groups(P>0.05).The HVC group had a signifi cantly lower bone cement leakage rate and a higher excellent/good bone cement distribution rate than the CVC group(P<0.05).The HVC group showed more signifi cant improvement in VAS scores at 72 hours postoperatively and lower ODI scores at 3 months postoperatively(P<0.05).No signifi cant difference was found in complication rates between groups(P>0.05).High-viscosity bone cement signifi cantly reduces the risk of bone cement leakage during PVP,with superior short-term pain relief and functional recovery,making it an effective choice for treating OVCF.
