摘要
目的基于三因司天方的临证使用,浅探顾植山教授“三虚”理论处方策略。方法整理顾植山教授2018年—2020年在无锡市中医医院门诊使用三因司天方的病案,以处方为单位对其所用之方进行分析。结果顾植山教授2018年—2020年处方包含三因司天方的病例数分别为248、223、147例,2018年使用频次最高的处方为静顺汤、麦门冬汤、川连茯苓汤,2019为敷和汤、白术厚朴汤、川连茯苓汤,2020年为正阳汤、牛膝木瓜汤、川连茯苓汤。使用频次最高的处方中,2018年静顺汤的患者的出生时间天干最多为戊、丁,地支最多为戌、辰;2019年敷和汤的天干最多为己、甲,地支最多为亥、巳;2020年正阳汤的天干最多为壬、戊,地支最多为午、子。结论顾植山教授在“三虚”致病理论的基础上,将辨运气之时、辨运气体质、辨运气之象三者结合,通过使用三因司天方调“天-人-邪”三者关系,从而达到治病的效果。其思路明确、特色鲜明,值得进一步学习。
Objective To analyze Professor Gu Zhishan’s prescription strategy based on the“theory of three deficiencies”in the clinical application of Sanyin Sitian prescription.Methods By sorting out the medical records of Professor Gu Zhishan’s use of Sanyin Sitian prescription in Wuxi Hospital of Traditional Chinese Medicine,the prescriptions used by Professor Gu Zhishan were analyzed in units of prescriptions.Results From 2018 to 2020,the number of cases in the prescriptions of Professor Gu Zhishan that included Sanyin Sitian prescription were 248,223,and 147 cases respectively.In 2018,the prescriptions with the highest frequency of use were Jingshun decoction,Maimendong decoction,and Chuanlian Fuling decoction.In 2019,they were Fuhe decoction,Baizhu Houpo decoction,and Chuanlian Fuling decoction.In 2020,they were Zhengyang decoction,Niuxi Mugua decoction,and Chuanlian Fuling decoction.Among the prescriptions with the highest frequency of use,in 2018,for the patients taking Jingshun decoction,the most common heavenly stems of their birth time were Wu and Ding,and the most common earthly branches were Xu and Chen.In 2019,for the patients taking Fuhe decoction,the most common heavenly stems were Ji and Jia,and the most common earthly branches were Hai and Si.In 2020,for the patients taking Zhengyang decoction,the most common heavenly stems were Ren and Wu,and the most common earthly branches were Wu and Zi.Conclusion Based on the pathogenic theory of“three deficiencies”,Professor Gu Zhishan combines the identification of the time of qi transformation and the five evolutive phases,the identification of the constitution related to qi transfor-mation and the five evolutive phases,and the identification of the manifestations of qi transformation and the five evolutive phases.By using Sanyin Sitian prescription to regulate the relationship among“heaven-human-pathogen”,the curative effect can be achieved.Professor Gu’s thinking is clear and his characteristics are distinct,which is worthy of further study.
