摘要
目的观察疏肝解郁化痰汤对甲状腺良性结节热消融术后患者甲状腺功能及复发的影响。方法选取2021年2月至2022年2月南京市中西医结合医院68例甲状腺良性结节患者为研究对照,按照随机数表法分为对照组和观察组,每组34例。对照组单纯行甲状腺良性结节热消融术治疗,观察组在热消融术后服用疏肝解郁化痰汤。比较2组临床疗效、中医症候评分、甲状腺功能及随访1年疾病复发情况。结果术后3个月,观察组、对照组总有效率分别为82.35%、70.59%,组间差异无统计学意义(χ^(2)=1.308,P=0.253)。术后3个月,2组主症评分、次症评分均显著下降(P<0.05),且观察组主症评分、次症评分均低于对照组(P<0.05)。术后3个月,2组促甲状腺素(TSH)水平均明显升高(P<0.05),游离三碘甲腺原氨酸(FT3)、游离甲状腺素(FT4)水平明显降低(P<0.05),且观察组TSH水平高于对照组(P<0.05),FT3、FT4水平低于对照组(P<0.05)。随访1年,均无失访病例,对照组复发8例(23.53%),观察组复发1例(2.94%),观察组复发率低于对照组(χ^(2)=4.610,P=0.032)。结论甲状腺良性结节热消融术后服用疏肝解郁化痰汤,可减轻患者临床症状,促进甲状腺功能恢复,降低结节复发率。
Objective To analyze the effect of Shugan Jieyu Huatan Tang on the recovery of thyroid function and recurrence rate in patients with benign thyroid nodules after thermal ablation surgery.Methods 68 patients with benign thyroid nodules(visiting from February 2021 to February 2022)were selected and divided into groups using the random number table method,with 34 patients in each group.The control group underwent simple thermal ablation of benign thyroid nodules,while the observation group took Shugan Jieyu Huatan Tang after thermal ablation.The clinical efficacy,traditional Chinese medicine syndrome scores,thyroid function indicators,and one-year disease recurrence were compared between the two groups.Results After 3 months of postoperative evaluation,the total effective rates of the observation group and the control group were 82.35%and 70.59%,with no significant difference between the groups(χ^(2)=1.308,P=0.253).After 3 months of surgery,the traditional Chinese medicine syndrome scores(main and secondary symptom scores)of the observation group were significantly lower than those of the control group;free triiodothyronine and free thyroxine were significantly lower than those of the control group,and thyroid stimulating hormone was significantly higher than that of the control group(P<0.05).After one year of follow-up,the recurrence rate of nodules in the observation group was 2.94%,which was significantly lower than that of the control group's 23.53%(χ^(2)=4.610,P=0.032).Conclusion Taking Shugan Jieyu Huatan Tang after thermal ablation of benign thyroid nodules can alleviate clinical symptoms,promote thyroid function recovery,and has outstanding value in reducing the risk of nodule recurrence,which is worth promoting.
