基于CBCT影像学评估上颌窦动脉发生率及骨孔的解剖学研究

摘要 目的运用CBCT研究上颌窦动脉(MSA)发生率及分型分布,观测其直径和走行位置并分析影响因素,探讨MSA对上颌后牙显微根尖手术的影响。方法回顾性分析2022年7月至2023年12月于首都医科大学附属北京安贞医院口腔科就诊的365例患者的CBCT影像,观测MSA发生率及分型分布,测量MSA骨孔直径(Da)、骨孔到上颌窦底距离(Df)和骨孔到牙槽嵴顶距离(Dc),分析其直径和走行的影响因素。结果MSA骨孔检出率88.56%,右侧(87.03%)较左侧(85.85%)检出率高(P<0.05),上颌第一前磨牙(P1)水平检出率最高,为94.11%,第二磨牙(M2)水平检出率最低,为81.64%。以血管数量分型,单血管检出率87.20%,三血管检出率1.47%,前磨牙区多血管检出率高。以血管走行分类,a型检出率最高为50.80%,c型最低为1.02%。Da为(1.03±0.43)mm,Da>2 mm组占比为4.38%。男性Df、Da更大(P<0.05);年龄与Da、Dc正相关(P<0.001);Df和Da负相关(P<0.001)。结论牙位靠后、年龄较长人群Da较大,女性和磨牙区Df更小,多血管吻合发生在前磨牙区频率更高,这些均为术中损伤MSA的危险因素,需在术前仔细判读、综合分析。 Objective To investigate the incidence and distribution patterns of the maxillary sinus artery(MSA),observe its diameter and course,analyze influencing factors,and explore the impact of MSA cone-beam computed tomography(CBCT).Methods A retrospective analysis was conducted on the CBCT of 365 patients visited the Department of Stomatology at Beijing Anzhen Hospital from July 2022 to December 2023.The incidence and distribution patterns of MSA were observed through CBCT images.The diameter of MSA,the distances from MSA to the floor of the maxillary sinus(Df)and the alveolar crest(Dc)were measured.Factors influencing the diameter and course of MSA were analyzed.Results The detection rate of MSA was 88.56%,with a higher rate on the right side(87.03%)compared to the left(85.85%).The highest detection rate was at the P1 level(94.11%)and the lowest was at the M2 level(81.64%).The single-vessel detection rate was 87.20%,while the triple-vessel detection rates was 1.47%.The frequency of multiple vessels increased with anterior positions.The detection rate for type a was 50.80%,and type c was the lowest at 1.02%.The Da was(1.03±0.43)mm,and 4.38%of cases with Da>2 mm.Male had larger Df and Da(P<0.05).Age was positively correlated with Da and Dc(P<0.001).Df and Da were negatively correlated(P<0.001).Conclusions The posterior tooth positions and older age tend to have larger Da.Female and molar region have smaller Df.Multiple vessel anastomoses occur more frequently in premolars.These factors are potential risks for MSA injury and require comprehensive analysis.
