摘要
目的分析2019—2023年漳州市HIV抗体不确定样本带型特征及随访结果,为HIV抗体不确定综合诊断提供参考。方法对2019—2023年漳州市收集到的经蛋白印迹(WB)试验检测判定为HIV抗体不确定的197份样本,分析带型分布特征,并对其中89例随访病例的酶联免疫法(ELISA)和胶体硒法(RT)试验结果及随访阳转情况进行分析。结果共2220份进行HIV抗体检测,其中197份HIV抗体结果判定为不确定,不确定率为8.95%。带型特征分析显示,出现频率居前3位的带型占78.68%(155/197),依次为p24占42.13%(83/197)、gp160占26.40%(52/197)、gp160+p24占10.15%(20/197)。在有效随访期内随访到89例,随访率为45.18%,其中41例阳转,随访阳转率为46.07%。HIV抗体复检试验呈现双反应且S/CO值≥6.00的样本阳转率为93.02%(40/43)。含gp160条带的样本随访阳转率71.70%(38/53),其中gp160+p24条带的样本随访阳转率为90.91%(10/11)。结论出现gp160提示感染风险较大,出现单一p24为非特异性反应可能性较大,HIV抗体复检试验出现双反应且S/CO值≥6.00,含有gp160的复合条带的HIV抗体不确定样本阳转率较高。建议对HIV抗体不确定人群随访检测和核酸试验进行补充试验纳入常规工作流程。
Objective To analyze the band pattern characteristics and follow-up results of HIV antibody indeterminate samples in Zhangzhou from 2019 to 2023 and provide a reference for the comprehensive diagnosls of HIV antibody indeterminate samples.Methods A total of 197 HIV antibody indeterminate samples,determined by western blot(WB)test in Zhang zhou from 2019 to 2023,were analyzed for band pattern distribution characteristics.The results of enzyme-linked immunosorbent assay(ELISA),colloidal selenium test(RT),and follow-up conversion of 89 cases were analyzed,along with the follow-up positive conversion rates.Results Among 2220 cases tested for HIV antibody,197 cases were judged to be indeterminate,with an uncertainty rate of 8.95%.Band characteristic analysis showed that the top three bands accounted for 78.68%(155/197)of the total frequency,in the following order:p24 accounted for 42.13%(83/197),gp160 accounted for 26.40%(52/197),and gp160+p24 accounted for 10.15%(20/197).During the effective follow-up period,89 cases were followed up,with a follow-up rate of 45.18%.Among them,41 cases converted to positive,with a follow-up positive conversion rate of 46.07%.The positive conversion rate of HIV antibody double reaction with S/CO value≥6.00 was 93.02%(40/43).The follow-up positive conversion rate for samples containing the gp160 band was 71.70%(38/53),and among these,the follow-up positive conversion rate for samples with both gp160+p24 bands was 90.91%(10/11).Conclusions The presence of gpl60 indicates a higher risk of infection,while the presence of single p24 is more likely to be a non-specific reaction.HIV antibody retesting with a double reaction and S/CO value≥6.00 shows a higher positive conversion rate.Indeterminate samples with multiple bands containing gpl60 have a higher positive conversion rate.It is recommended that follow-up testing and supplementary nucleic acid testing be included in the routine workflow for individuals with indeterminate HIV antibody results.
