摘要
精神健康社会工作是发展精神卫生事业的有力支撑。文章基于实务经验发现:由于“正常”化压力引发的自我耗竭和社会联结匮乏导致的支持不足,“熬”对精神障碍患者意味着身心双重的创伤、情理之间的张力和生死两端的博弈;当患者使用“有病”一词时,包含自嘲、获益与“反攻”三种动机;“怎么办”的求助体现了面对不可控的疾病,患者主动进行健康管理的能动性。在环境约束下通过动态的身份建构和灵活的行为选择,精神障碍患者发展出“做病人”的实践策略。在精神健康社会工作中,关注精神障碍患者日常表达的文化要素,有助于提升文化敏感性服务的质量,并为理论的本土化创新提供启发。
Mental health social work is a strong support for the development of mental health sector.Based on practical experience,this study explores the impact of the dual pressure of conforming to social norms and coping with a lack of emotional and social connection leading to insufficient support,the act of"enduring"for people with mental disorders often experience both physical and psychological trauma,the tension between rationality and emotion,as well as the struggle between survival and despair.The expression"being sick",carries complex layers of meaning.It can reflect self-deprecation,a way to gain understanding or support,or even a subtle form of resistance against societal stigma.Similarly,when patients ask"What should I do?",it reflects their effort to regain agency in the face of unpredictable illness,signaling an active pursuit of self-management rather than passive dependence.Faced with environmental constraints,people with mental disorders dynamically construct identities and adopt flexible behavioral choices,developing practical strategies of"performing patienthood".Attending to the cultural meanings in patients'daily expressions can enhance the cultural sensitivity of mental health social work and contribute to the localized development of relevant theories.
