摘要
目的:分析股骨粗隆间骨折透视牵引下股骨近端防旋髓内钉(PFNA)内固定术的效果。方法:选取2022年1月~2024年1月本院股骨粗隆间骨折患者60例,依据手术方法分为透视牵引下PFNA内固定术组(简称透视牵引下组)、动力髋螺钉内固定术组(简称动力髋螺钉组)两组,各30例。统计分析两组围术期指标、疼痛程度、髋关节功能、生活质量、炎症因子指标、临床疗效、术后并发症发生情况。结果:透视牵引下组患者的术中出血量少于动力髋螺钉组,切口长度短于动力髋螺钉组,手术时间、术后至下床时间、住院时间均短于动力髋螺钉组,骨痂出现时间短于动力髋螺钉组,负重时间短于动力髋螺钉组,骨折愈合时间短于动力髋螺钉组,数字分级法评分、白细胞计数、降钙素原、C反应蛋白水平均低于动力髋螺钉组,髋关节功能评分(Harris评分)、生活质量综合评定问卷(GQOL-74)评分、髋关节功能恢复优良率均高于动力髋螺钉组,术后并发症发生率6.67%(2/30)低于动力髋螺钉组26.67%(8/30)(χ^(2)=4.320,P<0.05)。结论:股骨粗隆间骨折治疗中透视牵引下PFNA内固定术的临床效果较动力髋螺钉内固定术好。
Objective:To analyze the effect of PFNA internal fixation under fluoroscopy traction for intertrochanteric fractures of the femur.Methods:60 patients with intertrochanteric fractures of the femur were selected and divided into two groups based on surgical methods:the PFNA internal fixation group under fluoroscopy traction(fluoroscopy traction group)and the dynamic hip screw internal fixation group(dynamic hip screw group),with 30 cases in each group.Statistical analysis of two groups of perioperative indicators,pain level,hip joint function,quality of life,inflammatory factor indicators,clinical efficacy,and incidence of postoperative complications.Results:Patients in the fluoroscopy traction group had less intraoperative bleeding than those in the dynamic hip screw group,shorter incision length,shorter surgical time,postoperative to bed time,and hospital stay than those in the dynamic hip screw group.The time of bone callus appearance was shorter than that in the dynamic hip screw group,the weight bearing time was shorter than that in the dynamic hip screw group,and the fracture healing time was shorter than that in the dynamic hip screw group.The NRS score,WBC,PCT,and CRP levels were all lower than those in the dynamic hip screw group.Harris score,GQOL-74 score,and hip joint function recovery rate were all higher than those in the dynamic hip screw group.The incidence of postoperative complications was 6.67%(2/30)lower than that in the dynamic hip screw group 26.67%(8/30)(χ^(2)=4.320,P<0.05).Conclusion:The clinical effect of PFNA internal fixation under fluoroscopy traction in the treatment of intertrochanteric fractures of the femur is better than that of dynamic hip screw internal fixation.
