摘要
目的分析床旁足浴清创联合脂质水胶体硫酸银敷料治疗WagnerⅡ~Ⅳ级糖尿病足病溃疡患者的临床效果。方法选取长春骨伤医院手足显微外科2022年3月~2023年6月收治的72例WagnerⅡ~Ⅳ级糖尿病足病患者,随机分为试验组(36例)和对照组(36例),试验组床旁足浴清创联合脂质水胶体硫酸银敷料治疗,对照组患者采用常规清创联合传统换药治疗,比较两组治疗后的临床效果。结果试验组创面愈合时间、炎症因子水平、疼痛评分、睡眠质量评分与对照组比较较低,结果差异有统计意义(P<0.05);试验组治疗2w后肉芽组织覆盖率、细菌清除率、创面面积缩小率与对照组比较改善效果更明显,结果差异有统计意义(P<0.05);两组ESR水平无统计意义(P>0.05);试验组治疗临床有效率明显高于对照组(94.44%vs 77.78%),差异具有统计意义(P<0.05)。结论床旁足浴清创联合脂质水胶体硫酸银敷料能提高WagnerⅡ~Ⅳ级糖尿病足病溃疡的创面愈合,降低血清炎症水平,减轻患者的疼痛感,患者的睡眠质量较高,临床疗效显著。
Objective To analyze the clinical effect of bedside foot bath debridement combined with lipid hydrocolloid silver sulfate dress-ing in the treatment of patients with Wagner grade II to IV diabetic foot disease ulcers.Method Seventy-two patients with Wagner grade II~IV diabetic foot disease admitted to the hand and foot microsurgery department of Changchun Orthopaedic Injury Hospital from March 2022 to June 2023 were selected and randomly divided into the experimental group(36 cases)and the control group(36 cases),and the experimental group was treated with bedside foot bath debridement combined with lipohydrocolloid silver sulfate dressing,while the patients in the control group were treated with conventional debridement combined with conventional dressing change,and the clinical effect of the treatment in these two groups was compared.Compare the clinical effects of the two groups after treatment.Result The wound healing time,inflammatory factor level,pain score and sleep quality score of the experimental group were lower than those of the control group,and the difference was statistically significant(P<0.05);the improvement of the granulation tissue coverage rate,bacterial clearance rate,and wound area reduction rate of the experimental group was more obvious compared with those of the control group after 2 w of treatment,and the difference was statistically significant(P<0.05);there was no statistically significant difference in the ESR level of the two groups(P>0.05);the ESR level of the two groups was not statistically significant.(P>0.05);the clinical effectiveness rate of the experimental group was significantly higher than that of the control group(94.44%vs 77.78%),and the difference was statistically significant(P<0.05).Conclusion Bedside footbath debridement combined with lipohydrocolloid silver sulfate dressing can improve wound healing of Wagner grade II-IV dia-betic podiatric ulcers,reduce serum inflammation level,alleviate patients'pain,patients'sleep quality is higher,and the clinical efficacy is remarkable.
