膝关节前交叉韧带损伤患者MRI影像特点及对膝关节稳定性的评估价值

摘要 目的探究膝关节前交叉韧带(ACL)损伤患者磁共振成像(MRI)影像特点及对膝关节稳定性的评估价值。方法选择98例膝关节ACL损伤患者作为研究对象。所有患者均接受MRI检查,比较不同膝关节ACL损伤程度患者MRI影像学特点与膝关节稳定性,采用Pearson相关性分析MRI影像学指标与膝关节稳定性的关系。结果ACL损伤程度:1级57例、2级41例。ACL损伤1级的后交叉韧带近端与远端形成的夹角(PCL角)、ACL与胫骨内侧平台在矢状面的夹角(ACL角)和Lysholm、国际膝关节文献委员会评分(IKDC)、伯格平衡量表(BBS)评分明显高于ACL损伤2级患者(t分别=5.82、8.48、6.52、8.51、4.02,P均<0.05),Blumensaat角、ACL前内侧长度、ACL厚度明显低于ACL损伤2级患者(t分别=5.88、3.29、2.43,P均<0.05)。Pearson相关性分析结果显示,PCL角、ACL角均与Lysholm评分、IKDC评分、BBS评分呈正相关(r分别=0.64、0.73、0.50;0.65、0.84、0.58,P均<0.05);Blumensaat角、ACL前内侧长度、ACL厚度均与Lysholm评分、IKDC评分、BBS评分呈负相关(r分别=-0.70、-0.82、-0.63;-0.46、-0.51、-0.40;-0.43、-0.48、-0.36,P均<0.05)。结论ACL损伤患者MRI影像学指标与膝关节稳定性具有一定相关性,可用于损伤诊断和损伤程度评估。 Objective To explore imaging characteristics of magnetic resonance imaging(MRI)and their evaluation value for knee stability in patients with knee anterior cruciate ligament(ACL)injury.Methods A total of 98 patients with knee ACL injury were enrolled as the research objects,and all underwent MRI examination.The imaging characteristics of MRI and knee stability in patients with different severity of ACL injury were compared.The relationship between MRI imaging indexes and knee stability was analyzed by Pearson correlation analysis.Results There were 57 cases with ACL injury of degree 1 and 41 cases of degree 2.The proximal and distal intersection angle of posterior cruciate ligament(PCL angle),sagittal intersection angle between ACL and medial tibial platform(ACL angle),scores of Lysholm,the international knee documentation committee knee evaluation form(IKDC)and Berg balance scale(BBS)in patients with ACL injury of degree 1 were significantly higher than those of degree 2(t=5.82,8.48,6.52,8.51,4.02,P<0.05),while Blumensaat angle,length of anterior medial ACL and ACL thickness were significantly lower than those of degree 2(t=5.88,3.29,2.42,P<0.05).The pearson correlation analysis showed that PCL angle and ACL angle were positively correlated with scores of Lysholm,IKDC and BBS(r=0.64,0.73,0.50,0.65,0.84,0.58,P<0.05).The Blumensaat angle,length of anterior medial ACL,and ACL thickness were negatively correlated with them(r=-0.70,-0.82,-0.63,-0.46,-0.51,-0.40,-0.43,-0.48,-0.36,P<0.05).Conclusion MRI imaging indexes have certain correlation with knee stability in patients with ACL injury,which can be applied in diagnosis and severity evaluation of the injury.
