摘要
目的分析德阳市2019—2023年其他感染性腹泻流行特征、病原分型,探讨发病的时空聚集点,识别潜在的聚集风险,为风险研判、预警和防控提供科学依据。方法采用描述性分析方法分析2019—2023年德阳市其他感染性腹泻的流行特征,率的比较使用SPSS 25.0软件进行χ^(2)检验,运用ArcGIS 10.6软件进行空间聚集性分析,使用SATSCAN 9.5软件进行时空扫描分析,检验水准为α=0.05。结果2019—2023年德阳市累计报告其他感染性腹泻8489例,年均发病率48.59/10万,各年度间报告发病率呈上升趋势(χ_(趋势)^(2)=780.82,P<0.001),每年冬季和夏季2个高峰,地区间发病水平差异明显,易感人群以5岁以下儿童为主。有明确病原学诊断的病例中,病毒性腹泻感染占85.01%。聚集性疫情以学校诺如病毒感染为主,空间自相关显示2019—2023年德阳市其他感染性腹泻发病率空间上无显著的聚类和空间相关性,时空扫描显示主要集聚区为旌阳区、罗江区和聚集时间为2023年6月至2023年12月。结论2019—2023年德阳市其他感染性腹泻疫情总体呈上升趋势,表现为较明显的季节高发、地区高发和人群高发,5岁以下散居儿童是主要的易感人群,学校聚集性疫情是防控工作的重点,应采取综合干预措施控制暴发和流行。
Objective To analyze the epidemiological characteristics and pathogen types of other infectious diarrhea in Deyang City from 2019 to 2023,to explore the temporal and spatial aggregation points of morbidity,to identify potential aggregation risks,and to provide scientific basis for risk research,early warning,prevention and control.Methods Descriptive analysis was used to analyze the epidemiological characteristics of other infectious diarrhea in Deyang City from 2019 to 2023,and comparisons of rates were made by Chi-square test with SPSS 25.0.ArcGIS 10.6 was used for spatial clustering analysis,and SATSCAN 9.5 for spatiotemporal scanning.And the test level wasα=0.05.Results From 2019 to 2023,Deyang City cumulatively reported 8489 cases of other infectious diarrhea,with an average annual incidence rate of 48.59/105,and with an increasing trend in the reported incidence rate among years(χ_(trend)^(2)=780.82,P<0.001).There were two peaks in winter and summer every year,and the incidence level varied significantly among regions.The susceptible population was dominated by children under 5 years old.Among the cases with definite etiological diagnosis,viral diarrhea infection accounted for 85.01%.The cluster was dominated by norovirus infection in schools.Spatial autocorrelation showed no significant clustering and spatial correlation in the incidence of other infectious diarrhea in Deyang City from 2019 to 2023.The spatiotemporal scanning showed that the main cluster areas were Jingyang District and Luojiang District,and the agglomeration period was from June 2023 to December 2023.Conclusions From 2019 to 2023,the incidence of other infectious diarrhea in Deyang City showed an overall upward trend,with obvious seasonal,regional and population high incidence.Scattered children under 5 years old were the main vulnerable groups,and cluster outbreaks in schools were the focus of prevention and control work.Comprehensive intervention measures should be taken to control the outbreak and epidemic.
