摘要
目的:观察富血小板纤维蛋白联合牙髓血管再生术治疗根尖周炎患者的效果。方法:选取2021年8月至2023年8月该院收治的80例根尖周炎患者进行前瞻性研究,按照随机数字表法将其分为对照组与研究组各40例。对照组给予牙髓血管再生术治疗,研究组在对照组基础上联合富血小板纤维蛋白治疗。比较两组临床疗效,治疗前后牙周指标[菌斑指数(PLI)、牙龈指数(GI)、出血指数(BI)]、咀嚼效率、咬合力、炎性因子[超敏C反应蛋白(hs-CRP)、白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)]水平,以及并发症发生率。结果:研究组治疗总有效率为97.50%(39/40),高于对照组的80.00%(32/40),差异有统计学意义(P<0.05);治疗后,两组PLI、GI、BI均低于治疗前,且研究组低于对照组,差异有统计学意义(P<0.05);治疗后,两组咀嚼效率和咬合力水平均高于治疗前,且研究组高于对照组,差异有统计学意义(P<0.05);治疗后,两组hs-CRP、IL-8、TNF-α水平均低于治疗前,且研究组低于对照组,差异有统计学意义(P<0.05);两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:富血小板纤维蛋白联合牙髓血管再生术治疗根尖周炎患者可提高治疗总有效率、咀嚼效率和咬合力水平,降低牙周指标和炎性因子水平,效果优于单纯牙髓血管再生术治疗。
Objective:To observe effects of platelet-rich fibrin combined with dental pulp vascular regeneration in treatment of periapical periodontitis.Methods:A prospective study was conducted on 80 patients with periapical periodontitis admitted to the hospital from August 2021 to August 2023.According to the random number table method,they were divided into control group and study group,40 cases in each group.The control group was treated with dental pulp vascular regeneration,while the study group was treated with platelet-rich fibrin on the basis of that of the control group.The clinical effects,the periodontal indexes[plaque index(PLI),gingival index(GI),bleeding index(BI)]levels,the masticatory efficiency and bite force levels,the inflammatory factors[high-sensitivity C-reactive protein(hs-CRP),interleukin-8(IL-8),tumor necrosis factor-α(TNF-α)]levels before and after the treatment,and the incidence of complications were compared between the two groups.Results:The total effective rate of the study group was 97.50%(39/40),which was higher than 80.00%(32/40)of the control group,and the difference was statistically significant(P<0.05).After the treatment,the levels of PLI,GI and BI in the two groups were lower than those before the treatment,those in the study group were lower than the those in control group,and the differences were statistically significant(P<0.05).After the treatment,the masticatory efficiency and bite force levels of the two groups were higher than those before the treatment,those of the study group were higher than those of the control group,and the differences were statistically significant(P<0.05).After the treatment,the levels of hs-CRP,IL-8 and TNF-αin the two groups were lower than those before the treatment,those in the study group were lower than those in the control group,and the differences were statistically significant(P<0.05).However,there was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusions:Platelet-rich fibrin combined with dental pulp vascular regeneration in the treatment of periapical periodontitis can improve the total effective rate and the masticatory efficiency and bite force levels,and reduce the periodontal index levels and the inflammatory factor levels.Moreover,it is superior to simple dental pulp vascular regeneration.
