摘要
目的探讨自动乳腺全容积成像系统(ABVS)的冠状面声像图对BI-RADS 4类肿瘤再评级与特征性分析的临床价值。方法选取常规超声诊断BI-RADS 4类277例病灶,行ABVS二次评分评级,并与对应病理做对照,比较ABVS冠状面评级中前后邻近两级,计算ABVS冠状面评级与常规超声BI-RADS分级对乳腺良恶性诊断的灵敏性、特异性、准确性、阳性预测率及阴性预测率。结果277例BI-RADS 4类肿瘤良性193例、恶性84例,4A类141例、4B类78例、4C类58例,ABVS冠状面评级0~5级分别有5例、50例、64例、72例、51例、25例,1级、2级、3级对良恶性诊断效果无统计学差异,4级与5级对良恶性诊断效果无统计学差异。常规超声与ABVS冠状面对BI-RADS4类肿块诊断的灵敏度、特异度、准确性分别为71.4%vs 88.1%、74.6%vs 86.0%、73.6%vs 86.6%、70.2%vs 88.1%、73.6%vs 85.0%,差异均有统计学意义。结论ABVS冠状面声像图能提高诊断BI-RADS 4类亚分类肿瘤良恶性的准确性,比常规超声更具鉴别诊断能力。
Objective To investigate the clinical value of coronal ultrasonography of automatic breast full-volume imaging system for re-rating and characteristic analysis of BI-RADS type 4 tumors.Methods A total of 277 patients with BI-RADS type 4 lesions were selected for conventional ultrasound diagnosis,and ABVS secondary grading was performed,and compared with the corresponding pathology,and the two grades of ABVS coronal plane grading were compared.The sensitivity,specificity,accuracy,positive prediction rate and negative prediction rate of ABVS coronal surface rating and conventional ultrasound BI-RADS grading for benign and malignant breast diagnosis were calculated.Results Among 277 BI-RADS 4 tumors,193 were benign and 84 were malignant,141 were 4A,78 were 4B,58 were 4C.There were 5 cases,50 cases,64 cases,72 cases,51 cases and 25 cases with ABVS coronal surface rating from 0 to 5,respectively.There was no statistical difference in benign and malignant diagnosis effect of GRADE 1,grade 2,and grade 3.There was no statistical difference between grade 4 and grade 5 in benign and malignant diagnosis.The sensitivity,specificity and accuracy of conventional ultrasound and ABVS coronal diagnosis of BI-RADS4 mass were 71.4%vs 88.1%,74.6%vs 86.0%,73.6%vs 86.6%,70.2%vs 88.1%,73.6%vs 85.0%,respectively.The differences were statistically significant.Conclusion ABVS coronal ultrasonography can improve the accuracy of diagnosis of bi-RADS 4 subclass tumor benign and malignant,and has more differential diagnosis ability than conventional ultrasonography.
