摘要
目的旨在探讨患者输注异体红细胞(RBC)后血红蛋白(Hb)与红细胞分布宽度(RDW)比值(HRR)的变化。方法根据单中心回顾性研究,纳入2022年3月—2024年1月在医院申请输注一袋去白细胞悬浮红细胞的213例患者,分析其输血前(T0)、输血后1天(T1)、输血后7天(T2)、输血后15天(T3)和输血后1个月(T4)的HRR值,并收集患者基本临床资料及其试验数据。根据患者的外周血Hb与RDW计算HRR值。ΔHRR表示输血后各时间点的HRR与T0时HRR的差值。结果输血后各时间点的HRR值均升高(P<0.05),其中T4时最高。通过亚组分析发现,对比T0时正常平均红细胞体积(MCV)组和异常MCV组,前者的最大ΔHRR更大(P<0.05);对比T0时正常RDW组和异常RDW组,两者的最大ΔHRR差异无统计学意义(P=0.163);对比T0时低水平Hb浓度和高水平Hb浓度,两者的最大ΔHRR差异无统计学意义(P=0.340);对比T0时肿瘤组和非肿瘤组,两者的最大ΔHRR差异无统计学意义(P=0.264)。结论输注RBC可以增加HRR,在使用HRR作为预后指标时应考虑输血的影响。
Objective To investigate the changes in hemoglobin-to-red cell distribution width ratio(HRR)in the patients after transfusion of allogeneic red blood cells(RBCs).Methods This was a single-center retrospective study that included 213 patients who applied for transfusion of one bag of deleterious leukocyte suspension erythrocytes at our hospital from March 2022 to January 2024 were included,and their HRR values were analyzed before the transfusion(T0),1 day after the transfusion(T1),7 days after the transfusion(T2),15 days after the transfusion(T3)and 1 month after the transfusion(T4),and the basic clinical information of the patients and their experimental data were collected.HRR values were calculated from the patient's peripheral hemoglobin(Hb)and red blood cell distribution width(RDW),andΔHRR was calculated as the difference between the HRR at each time point after transfusion and the HRR at T0.Results The HRR values were elevated at all time points after blood transfusion(P<0.05),with the highest at T4.By subgroup analysis,it was found that comparing the normal MCV group and the abnormal MCV group at T0,the maximumΔHRR of the former was greater(P<0.05);comparing the normal RDW group and the abnormal RDW group at T0,the difference in the maximumΔHRR of the two was not statistically significant(P=0.163);comparing the low level of Hb concentration and the high level of Hb concentration at T0,the difference in the maximumΔHRR of the two was not statistically(P=0.340);comparing the tumor group and non-tumor group at T0,the difference in maximumΔHRR between the two was not statistically significant(P=0.264).Conclusion Transfusion of erythrocytes can increase the HRR value,and the effect of blood transfusion should be considered when using HRR as a prognostic index.
