摘要
目的 探讨密闭式粉碎袋在腹腔镜子宫肌瘤剔除术中的应用。方法 选取2022年6月至2023年6月于乐平市人民医院行子宫肌瘤剔除术的80例患者作为研究对象,按照随机数字表法分为观察组与对照组,每组40例。对照组术中采用传统腹腔镜取物袋取出瘤体,观察组术中采用密闭式粉碎袋取出瘤体。比较两组粉碎袋放置时间、瘤体粉碎及瘤体取出时间、整体手术时间、术中出血量、术后切口疼痛评分、标本取出重量、手术并发症发生情况、术后冲洗液梭形细胞检测阳性情况、术后1年复发情况。结果 两组病灶粉碎时间、瘤体取出时间、粉碎袋放置时间、整体手术时间、术中出血量、术后切口疼痛评分及标本取出重量比较差异无统计学意义。观察组术后并发症发生率、术后冲洗液梭形细胞阳性检出率及术后1年复发率均低于对照组,差异有统计学意义(P<0.05)。结论 密闭式粉碎袋用于腹腔镜子宫肌瘤剔除术患者中,能有效避免术中操作时散落的肿瘤细胞进入腹腔内发生散播种植,有效预防子宫肌瘤的复发,且操作简单、安全可靠,不会引发术后相关并发症,值得临床推广应用。
Objective To explore the application of closed fragmentation bag in laparoscopic hysteromyomectomy.Methods 80 patients who under-went myomectomy in Leping People!s Hospital from June 2022 to June 2023 were selected as the study subjects,they were divided into the observation group and the control group according to the random number table method,with 40 cases in each group.The control group used the traditional laparoscopic to remove the tumor,while the observation group used self-made sealed fragmentation bag to remove the tumor.The lesion fragmentation time,tumor remov-al time,bag removal time,overall surgical time,intraoperative bleeding,the postoperative incision pain score,the sample removal weight,postoperative com-plications and positive detection rate of spindle cell in postoperative flushing fluid of and recurrence one year after operation were compared between the two groups.Results There were no significant differences in the lesion fragmentation time,tumor removal time,bag removal time,overall surgical time,in-traoperative bleeding,the postoperative incision pain score and the sample removal weight between the two groups.The incidence of postoperative complica-tions,the positive detection rate of spindles cell and the recurrence rate one year after operation in the observation group were lower than those in the con-trol group,and the differences were statistically significant(P<0.05).Conclusion The use of closed fragmented bags in patients undergoing laparoscopic uterine fibroidectomy can effectively prevent scattered uterine fibroid cells from entering the abdominal cavity during the operation,effectively preventing uterine fibroid recurrence,and the operation is simple,safe and reliable,without causing related postoperative complications,which is worthy of clinical pro-motion and application.
