摘要
目的:分析CT三维重建在L5/S1腰椎间盘突出症椎间孔镜手术入路中的临床应用。方法:选取2019年1月至2020年12月在新昌县人民医院诊治的60例腰椎间盘突出症患者,按随机数字表法将其分为A组(n=30)和B组(n=30)。A组患者采用CT重建导航后行椎间孔镜手术,B组患者采用常规CT检查后行椎间孔镜手术。比较两组手术指标、疼痛程度、腰椎功能状况。结果:A组工作通道建立时间、手术时间、穿刺次数及术中失血量低于B组(P<0.05);与术前相比,两组术后的疼痛视觉模拟评分和Oswestry功能障碍指数评分降低(P<0.05);术后,两组患者的腰椎结构参数较术前整体改善,且B组优于A组(P<0.05)。结论:无论是常规CT检查后行椎间孔镜术的L5/S1腰椎间盘突出症患者,还是三维CT导航后行椎间孔镜术的L5/S1腰椎间盘突出症患者,其术后疼痛程度、肢体功能症状、腰椎结构均有所改善,且疗效相似,但后者术中工作通道建立时间和手术时间更短,术中穿刺次数和术中出血量更少。
Objective:To analyze the clinical efficacy of CT 3D reconstruction in L5/S1 lumbar disc herniation.Methods:60 patients with lumbar disc herniation diagnosed and treated in Xinchang County Peoples Hospital from January 2019 to December 2020 were selected and divided into group A(n=30)and Group B(n=30)according to random chart.Patients in group A underwent foraminal surgery after CT reconstruction navigation,and patients in group B underwent foraminal surgery after conventional CT examination.Surgical index,pain level,and lumbar functional status were compared between the two groups.Results:The working channel establishment time,operation time,puncture times and intraoperative blood loss in Group A were lower than Group B(P<0.05),postoperative pain visual analog score and Oswestry dysfunction index score decreased(P<0.05);after surgery,the lumbar structure parameters in both groups improved compared with preoperative,and group B was better than Group A(P<0.05).Conclusion:Whether L5/S1 patients with foraminal disc herniation after conventional CT examination or L5/S1 patients with foraminal disc herniation after 3 D CT navigation,the postoperative pain degree,limb function symptoms and lumbar structure improved with similar efficacy.However,the latter had shorter establishment time and operation time,less intraoperative puncture times and intraoperative bleeding.
