摘要
目的探究纤溶酶联合脉络舒通颗粒治疗下肢深静脉血栓(lower limb deep vein thrombosis,LEDVT)患者的临床效果及对血清溶血磷脂酸(lysophosphatidic acid,LPA)、血管性血友病因子(vascular hemophilia factor,vWF)水平的影响。方法该研究对象选取的是2017年2月—2018年2月的LEDVT患者共计90例,应用完全随机性原则分组,具体方法为随机数字表法。对照组45例患者的治疗用药为纤溶酶,观察组为纤溶酶+脉络疏通颗粒。比较两组患者临床疗效和不良反应,对两组患者治疗前后凝血功能指标[纤维蛋白原(fibrinogen,FIB)、凝血酶原时间(prothrombin time,PT)、D-二聚体(D-Dimer)、活化部分凝血活酶时间(activated partial thromboplastin time,APTT)]水平以及血清LPA、vWF水平进行检测和对比。结果观察组的有效率显著高于对照组(P<0.05)。治疗前凝血功能指标在两组患者之间对比差异无统计学意义(P>0.05),治疗后FIB和D-Dimer降低,PT和APTT升高(P<0.05),治疗后观察组PT、APTT显著低于对照组而FIB和D-Dimer显著高于对照组(P<0.05)。治疗前两组血清LPA、vWF水平比较差异无统计学意义(P>0.05),治疗后均显著降低(P<0.05),治疗后观察组血清LPA、vWF水平显著低于对照(P<0.05)。治疗不良反应方面两组患者之间对比差异无统计学意义,安全性相当(P>0.05)。结论纤溶酶联合脉络舒通颗粒对可显著改善LEDVT患者凝血功能指标,具有更好的临床效果,同时可以下调血清中LPA、vWF水平,具有更好的预后,并具有一定的安全性。
Objective To investigate the effect of fibrinolytic enzyme combined with Mailuo shutong Granule(脉络舒通颗粒)on the treatment of lower limb deep vein thrombosis(LEDVT)patients and its impact on serum levels of lysophosphatidic acid(LPA)and vascular hemophilia factor(vascular hemophilia factor,vWF).Method This study selected a total of 90 LEDVT patients from February 2017 to February 2018,and grouped them using the principle of complete randomness.The specific method was the random number table method.The treatment medication for 45 patients in the control group was plasmin,while the observation group was plasmin+Mailuo Shutong Granule.The clinical efficacy and adverse reactions were compared between two groups.The levels of coagulation function indicators,including fibrinogen(FIB),prothrombin time(PT),D-dimer(D-dimer,D-dimer),activated partial thromboplastin time(APTT),as well as serum LPA and vWF levels,were detected and compared before and after treatment in two groups.Results The effective rate of the observation group was higher than that of the control group(P<0.05).There was no significant difference in coagulation function indicators between the two groups before treatment(P>0.05).After treatment,FIB and D-Dimer decreased,while PT and APTT increased(P<0.05).After treatment,PT and APTT in the observation group were significantly lower than those in the control group,while FIB and D-Dimer were significantly higher than those in the control group(P<0.05).There was no significant difference in serum LPA or vWF levels between the two groups before treatment(P>0.05),but the levels in both groups significantly decreased after treatment(P<0.05).After treatment,the serum LPA and vWF levels in the observation group were significantly lower than those in the control group(P<0.05).There was no significant difference in the treatment of adverse reactions between the two groups,and the safety was comparable(P>0.05).Conclusion Fibrinolytic enzyme combined with Mailuo shutong Granule can significantly improve the coagulation function index of patients with LEDVT,and has better clinical effect.At the same time,it can down-regulate the levels of LPA and vWF in serum,have better prognosis and certain safety.
