摘要
目的:综合评价抗病毒治疗联合儿童乙肝免疫球蛋白(HBIG)注射对阻断乙型肝炎病毒(HBV)高病毒载量孕妇母婴传播的效果,为有针对性地对重点人群采取有效的综合干预措施阻断母婴传播提供科学依据。方法:检索中国知网、维普、万方等数据库,收集2015—2024年关于HBV高病毒载量孕妇采取综合干预措施阻断母婴传播效果评价的病例对照研究文献,选取干预组(抗病毒治疗+HBIG)与对照组(单纯HBIG)新生儿乙肝母婴传播阻断率作为效应量,应用Stata软件进行meta分析。结果:共纳入10篇符合标准的文献,合计样本量为2746例,文献分析结果显示干预组乙肝母婴传播阻断率高于对照组,差异有统计学意义(OR=7.32,P<0.05)。结论:对孕妇进行抗病毒治疗联合HBIG注射对HBV母婴阻断效果优于单纯注射HBIG。
OBJECTIVE To comprehensively evaluate the effect of antiviral treatment combined with children’s hepatitis B immunoglobulin injection on blocking mother to child transmission of hepatitis B virus in pregnant women with high viral load,and to provide scientific basis for targeted comprehensive intervention measures to block mother to child transmission of key populations.METHODS CNKI,CBM,Wanfang,VIP,PubMed databases were searched to collect and compare the randomized controlled trials of antiviral therapy combined with HBIG and simple HBIG to block the mother to child transmission of HBV high viral load pregnant women from 2015 to 2024.The blocking rate of mother to child transmission of hepatitis B in newborns in the combined intervention and simple intervention groups was selected as the effect amount,and the stata software was used for meta analysis.RESULTS A total of 10 studies that met the criteria were included,with a total sample size of 2746 cases.The results showed that the mother to child transmission blocking rate in the combined intervention group was higher than that in the simple intervention group(OR=7.32,P<0.05).CONCLUSION The maternal and infant blocking effect of antiviral therapy combined with HBIG in pregnant women is better than that of HBIG alone.
