摘要
目的比较高频彩色多普勒超声(简称“高频彩超”)与磁共振成像(MRI)对冈上肌钙化性肌腱炎患者的诊断价值。方法选取2023年5月至2024年5月医院收治的80例疑似冈上肌钙化性肌腱炎患者为研究对象,所有患者均接受高频彩超、MRI检查,以肩关节镜检查为金标准,比较2种检查方法与金标准的一致性和诊断效能。结果80例疑似患者经肩关节镜检查确诊62例为冈上肌钙化性肌腱炎;高频彩超检查结果与肩关节镜检查结果具有高度一致性;MRI检查结果与肩关节镜诊断结果具有中度一致性;2种方法的诊断特异度、阳性预测值比较,差异无统计学意义(P>0.05);高频彩超检查的诊断敏感度、准确度、阴性预测值均高于MRI,差异有统计学意义(P<0.05)。结论高频彩超诊断冈上肌钙化性肌腱炎的效能优于MRI,建议临床采用高频彩超作为诊断该病的首选工具。
Objective Comparison of the diagnostic value of high-frequency color Doppler ultrasound(high-frequency color ultrasound)and magnetic resonance imaging(MRI)in patients with supraspinatus calcific tendinitis.Methods 80 patients with suspected calcific tendinitis of supraspinatus admitted to the hospital from May 2023 to May 2024 were selected as the study subjects,and all of them were examined by high-frequency color ultrasound and MRI,and shoulder arthroscopy was used as the gold standard,so as to compare the consistency of the two methods of examination with the gold standard and diagnostic efficacy.Results A total of 80 suspected patients were examined by shoulder arthroscopy,and 62 cases were diagnosed as calcific tendinitis of supraspinatus muscle;the results of high-frequency color ultrasound and shoulder arthroscopy had high consistency;the results of MRI and shoulder arthroscopy had moderate consistency;the comparison of diagnostic specificity and positive predictive value between the two methods was not statistically significant(P>0.05);the diagnostic sensitivity,accuracy and negative predictive value were higher than that of MRI,and the difference was statistically significant(P<0.05).Conclusion The efficacy of high-frequency color ultrasound is superior to MRI in diagnosing calcific tendinitis of supraspinatus muscle,and high-frequency color ultrasound is recommended as the preferred diagnostic tool for diagnosing this disease in clinical practice.
