摘要
目的探究剖宫产术后应用舒芬太尼复合艾司氯胺酮的镇痛效果。方法选取2022年3月—2024年3月徐州市妇幼保健院行剖宫产术分娩的120例产妇为研究对象,根据不同用药方法分为对照组和观察组,每组60例。对照组术后予以舒芬太尼+托烷司琼镇痛,观察组术后予以舒芬太尼+托烷司琼+艾司氯胺酮镇痛。比较两组术后2 h、4 h、6 h、12 h、24 h的镇痛、镇静效果,并统计两组镇痛期间不良反应发生情况。结果术后2 h、4 h、6 h、12 h、24 h时,观察组疼痛评分分别为(2.23±0.36)分、(2.54±0.34)分、(3.25±0.23)分、(3.34±0.28)分、(3.23±0.27)分,均低于对照组的(2.51±0.39)分、(2.82±0.35)分、(3.40±0.25)分、(3.57±0.30)分、(3.48±0.29)分,差异均有统计学意义(t=4.086,4.445,3.420,4.341,4.887;P均<0.05)。术后2 h、4 h、6 h、12 h、24 h时,观察组镇静评分均高于对照组,差异均有统计学意义(P均<0.05)。两组镇痛期间不良反应总发生率对比,差异无统计学意义(P>0.05)。结论于剖宫产术后应用舒芬太尼复合艾司氯胺酮可获得更为理想的镇痛、镇静作用,且安全性较高。
Objective To explore the analgesic effect of sufentanil combined with esketamine after cesarean section.Methods A total of 120 parturients who underwent cesarean section in Xuzhou Maternal and Child Health Hospital from March 2022 to March 2024 were selected as the research objects.According to different medication methods,they were divided into control group and observation group,with 60 cases in each group.The control group was given sufentanil+tropisetron for analgesia after operation,and the observation group was given sufentanil+tropisetron+esketamine for analgesia after operation.The analgesic and sedative effects of the two groups at 2 h,4 h,6 h,12 h and 24 h after operation were compared,and the incidence of adverse reactions during analgesia was counted.Results At 2 h,4 h,6 h,12 h and 24 h after operation,the pain scores of the observation group were(2.23±0.36)points,(2.54±0.34)points,(3.25±0.23)points,(3.34±0.28)points,(3.23±0.27)points,respectively,which were lower than those of the control group(2.51±0.39)points,(2.82±0.35)points,(3.40±0.25)points,(3.57±0.30)points,(3.48±0.29)points,the differences were statistically significant(t=4.086,4.445,3.420,4.341,4.887;all P<0.05).At 2 h,4 h,6 h,12 h and 24 h after operation,the sedation score of the observation group were higher than that of the control group and the differences were statistically significant(all P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups during analgesia(P>0.05).Conclusion The application of sufentanil combined with esketamine after cesarean section can obtain more ideal analgesic and sedative effects,and has higher safety.
