摘要
介绍彭清华教授从“伏燥”辨治干燥综合征型干眼症的临床经验。认为本病的关键病机为燥热内伏,提出分期辨治,早期以外燥引动,伏燥窜络,津液不足为主,治以清疏燥热、滋养津液,方以沙参麦冬汤加减;中期以内燥锢结,气阴枯涸,营血虚滞为主,治以益气养阴、补血润燥,方以生脉散加减;晚期以肝肾亏耗,阴损及阳,燥瘀阻络为主,治以阴阳双补、化瘀润燥,方以右归丸合二至丸加减。
This paper summarizes Professor Peng Qinghua's clinical experience in staged differentiation and treatment of Sjögren's syndrome-related dry eye based on the"latent dryness"theory.The key pathogenesis of this condition is identified as the internal accumulation of dry heat.A staged differentiation and treatment approach is introduced.The early stage is characterized by external dryness triggering latent dryness,which disrupts the collaterals,leading to insufficient body fluids.The treatment focuses on clearing and dispersing dry heat while nourishing body fluids,using a modified Shashen Maidong Decoction.The middle stage is marked by the entrenched internal dryness,depletion of qi and yin,and deficiency and stagnation of nutrient-blood.The treatment emphasizes replenishing qi,nourishing yin,tonifying blood and moistening dryness,using a modified Shengmai Powder.The late stage is dominated by the exhaustion of liver and kidney,yin deficiency affecting yang,and dryness and stasis obstructing the collaterals.The treatment involves dual supplementation of yin and yang,resolving stasis,and moistening dryness,using a modified combination of Yougui Pill and Erzhi Pill.
