摘要
《金匮要略》对于痈脓的病机及发展规律灼有定见,临证上有其独到之处。张仲景四诊合参、谨守病机,结合肺痈、肠痈、狐惑等疾病不同阶段的特征性表现,从未成脓期、已成脓期两个不同期分别论治,创制了多首行之有效的经方。张仲景用药精当,善用桔梗祛痰排脓,适用薏苡仁祛湿消痈,巧用附子扶阳托毒,更用巴豆、贝母峻下祛邪,并用白芍、鸡子黄润养阴血。文章总结其辨治痈脓思路及用药规律,以期为临床治痈脓提供借鉴。
Synopsis of the Golden Chamber has a profound understanding of the pathogenesis and development of carbuncle,and has its own features in diagnosis and treatment.ZHANG Zhongjing used comprehensive analysis by the four examination methods and carefully observed of the pathogenesis,combined with the characteristic manifestations of different stages of lung carbuncle,intestinal carbuncle,Huhuo disease and other diseases.Treatments were separated from the two different stages of carbuncle:unpurulent and already purulent stages,and a number of classical prescriptions were created.ZHANG Zhongjing used medicinal herbs with precision,excelled in using Platycodonis Radix to expel phlegm and pus,and was suitable for using Coicis Semen to dispel dampness and carbuncle.He skillfully used Aconiti Lateralis Radix Praeparata to support yang and detoxify,and also used Crotonis Fructus and Fritillariae Thunbergi Bulbus to dispel evil spirits.He also used Paeoniae Radix Alba and egg yolk to nourish yin and blood.This article summarizes the ideas and medication rules for the diagnosis and treatment of carbuncle,in order to provide reference for clinical treatment of carbuncle.
