摘要
目的观察当归芍药散加减治疗高海拔地区高血压的临床效果。方法选择2024年3—11月青海省中医院收治的高海拔地区高血压患者100例,采用随机数字表法分为观察组和对照组,每组50例。对照组一级高血压患者服用苯磺酸氨氯地平片,二级高血压患者加用马来酸依那普利片治疗,观察组在对照组的基础上给予当归芍药散加减治疗,2周为1个疗程,2组均治疗2个疗程。比较2组临床疗效,治疗前后血压及其变异性指标[24 h平均收缩压(24 hSBP)、24 h收缩压变异性(24 hSBPV)、24 h平均舒张压(24 hDBP)、24 h舒张压变异性(24 hDBPV)]、中医症状积分、血脂指标[总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)],不良反应。结果观察组总有效率为98.00%,高于对照组的86.00%(χ^(2)=4.891,P=0.027)。治疗2个疗程后,2组24 hSBP、24 hSBPV、24 hDBP及24 hDBPV均明显下降,且观察组下降幅度大于对照组(P<0.01);2组主症、次症及总积分均明显下降,且观察组下降幅度大于对照组(P<0.01);2组TC、TG、LDL-C水平均明显下降,HDL-C水平均明显升高,且观察组下降或升高幅度大于对照组(P<0.01)。2组治疗期间均未出现皮疹、水肿、头痛、面部潮红等不良反应。结论当归芍药散加减治疗高海拔地区高血压临床疗效较好,显著降低患者血压,改善临床症状及血脂指标,且具有较高安全性。
Objective To observe the clinical effect of modified Danggui Shaoyao powder on hypertension in high-altitude area.Methods A total of 100 patients with hypertension in high altitude areas treated in Qinghai Provincial Hospital of Traditional Chinese Medicine from March to November 2024 were selected,and were divided into observation group and control group by random number table method,with 50 cases in each group.Control group was treated with amlodipine besylate tablets for primary hypertension and enalapril maleate tablets for secondary hypertension.Observation group was treated with modified Danggui Shaoyao powder on the basis of control group,2 weeks as a course of treatment,and two groups were treated with two courses of treatment.The clinical efficacy,blood pressure and its variability indexes(24 hSBP,24 hSBPV,24 hDBP,24 hDBPV),TCM symptom scores,blood lipid indexes(TC,TG,LDL-C,HDL-C)before and after treatment,adverse reactions between the two groups were compared.Results The total effective rate of the observation group was 98.00%,and that of the control group was 86.00%(χ^(2)=4.891,P=0.027).After two courses of treatment,24 hSBP,24 hSBPV,24 hDBP and 24 hDBPV in two groups were significantly decreased,and the decrease rate in observation group was greater than that in control group(P<0.01).The main disease,secondary disease and total score of 2 groups were significantly decreased,and the decrease rate of observation group was greater than that of control group(P<0.01).The levels of TC,TG and LDL-C were significantly decreased,and the levels of HDL-C were significantly increased in both groups,and the decrease or increase range in observation group was greater than that in control group(P<0.01).No adverse reactions such as rash,edema,headache and facial flushing occurred in two groups during treatment.Conclusion The addition and reduction of Danggui Shaoyao powder added or subtracted in the treatment of hypertension in high-altitude area can achieve good clinical efficacy,significantly reduce blood pressure,improve clinical symptoms and lipid indexes,and have high safety.
