摘要
目的 探讨椎体后凸成形术对骨质疏松性椎体压缩骨折患者椎旁肌退变及预后的影响。方法 收集2008年1月—2023年1月期间在天津市东丽医院进行治疗的180例骨质疏松性椎体压缩骨折患者的临床资料,将接受经皮椎体成形术的患者分为常规组(82例),将接受经皮椎体后凸成形术的患者分为观察组(98例)。比较两组患者手术相关指标、疼痛评分、腰痛功能、椎旁肌退变程度、近期预后和术后并发症。结果 观察组住院时间、骨密度、骨水泥分布优良率均优于常规组,差异有统计学意义(P<0.05);观察组术后疼痛评分、腰痛功能评分低于常规组,差异有统计学意义(P<0.05);观察组术后L3~S1各椎间盘水平椎旁肌横截面积高于常规组,差异有统计学意义(P<0.05);观察组术后椎体前缘高度高于常规组,而后凸Cobb角低于常规组,差异有统计学意义(P<0.05)。结论 椎体后凸成形术能明显缓解骨质疏松性椎体压缩骨折患者的临床症状,并恢复椎体前缘高度,短期预后好。
Objective To explore the effect of vertebral kyphoplasty on paraspinal muscle degeneration and prognosis in patients with osteoporotic vertebral compression fractures.Methods Clinical data of 180 patients with osteoporotic vertebral compression fractures treated in Dongli Hospital of Tianjin from January 2008 to January 2023 were collected.Patients who underwent percutaneous vertebroplasty were assigned to the control group(82 cases),while those who underwent percutaneous kyphoplasty were assigned to the observation group(98 cases).The surgical related indicators,pain scores,lower back pain function,degree of paraspinal muscle degeneration,recent prognosis,and postoperative complications of the two groups were compared.Results The hospitalization time,bone density,and excellent rate of bone cement distribution in the observation group were all better than those in the control group(P<0.05).The pain score and lower back pain function score after the operation in the observation group were significantly lower than those in the control group(P<0.05).The cross-sectional area of the paraspinal muscles at the level of intervertebral discs after the operation in the observation group L3~S1 was significantly higher than that in the control group(P<0.05).The height of the anterior vertebral body after the operation in the observation group was higher than that in the control group,and the Cobb angle of the posterior convexity was lower than that in the control group(P<0.05).Conclusion Posterior kyphoplasty can significantly alleviate the clinical symptoms of patients with osteoporotic vertebral compression fractures,restore the anterior vertebral height,and have good short-term prognosis.
