以兴奋性氨基酸/表皮生长因子受体通路为主探究三法三穴对坐骨神经轻度慢性压迫性损伤模型鼠的镇痛启动机制

To explore the analgesic initiation mechanism of three manipulations and three acupoints in minor CCI model mice through EAAT2/EGFR pathway

摘要 目的 观察三法三穴对坐骨神经轻度慢性压迫性损伤(minor chronic constriction injury, minor CCI)模型大鼠丘脑兴奋性氨基酸(recombinant excitatory amino acid transporter, EAAT2)/表皮生长因子受体(epidermal growth factor receptor, EGFR)通路的影响,探讨其镇痛作用及启动机制。方法 选取SPF级雄性SD大鼠32只,随机分为正常组、假手术组、模型组、推拿组,每组8只。模型组和推拿组建立minor CCI模型,推拿组于造模后于殷门穴、承山穴和阳陵泉穴进行1次点法、拨法和揉法干预。采用机械刺痛仪和热刺痛仪检测大鼠机械缩足反射阈值(mechanical withdrawal threshold, MWT)和热缩足反射阈值(thermal withdrawal latency, TWL),采用蛋白免疫印迹法检测大鼠丘脑EAAT2、谷氨酸(glutamic acid, Glu)、代谢型谷氨酸受体5(metabotropic glutamate receptor 5,mGluR5)、EGFR蛋白水平;反转录聚合酶链反应检测EAAT2、Glu、mGluR5、EGFR的mRNA水平。结果 造模后,与正常组和假手术组比较,模型组MWT和TWL显著降低(P<0.05),Glu、mGluR5、EGFR蛋白及mRNA水平显著升高(P<0.05),EAAT2蛋白及mRNA水平显著降低(P<0.05)。经三法三穴干预后,与模型组比较,推拿组MWT、TWL表达显著升高(P<0.05),Glu、mGluR5、EGFR蛋白及mRNA水平显著降低(P<0.05),EAAT2蛋白及mRNA水平显著升高(P<0.05)。结论 三法三穴可能通过减少EAAT2表达,从而抑制Glu与mGluR5的结合,降低mGluR5与EGFR的关联,抑制EGFR的表达及炎症反应,减少神经元细胞的损伤,从而达到镇痛的作用。 Objective The Recombinant Excitatory Amino Acid Transporter/Epidermal growth of minor chronic constriction injury(minor CCI)model was observed to study the influence of factor receptor(EAAT2/EGFR)pathway and explore its analgesic effect and its initiation mechanism.Methods Thirty-two male SPF SD rats were selected and randomly divided into normal group,sham group,model group and tuina group,with 8 rats in each group.The minor CCI model was established in the model group and the tuina group.After molding,the tuina group performed one intervention such as point method,pulling method and kneading method intervention at Yinmen point,Chengshan point and Yanglingquan point.The threshold values of mechanical foot shrinking reflex and thermal foot shrinking reflex were detected.The protein levels of Glutamic acid(Glu),metabotropic glutamate receptor 5(mGluR5)and EGFR in the thalamus of the rats were detected by Western blot,and the mRNA levels of Glu,mGluR5 and EGFR were detected by RT-PCR.Results After tuina intervention,compared with the normal group and the sham group,MWT and TWL values in the model group were significantly decreased(P<0.05),while Glu,mGluR5 and EGFR protein and mRNA levels were significantly increased(P<0.05)and EAAT2 protein and mRNA levels were significantly decreased(P<0.05).Compared with the model groups,the expressions of MWT and TWL in tuina group were significantly increased(P<0.05),while the protein and mRNA levels of Glu,mGluR5 and EGFR were significantly decreased(P<0.05),EAAT2 protein and mRNA levels were significantly increased(P<0.05),and the differences were statistically significant.Conclusion The three methods and three points may reduce the expression of EAAT2,thus inhibiting the combination of Glu and mGluR5,reducing the association between mGluR5 and EGFR,inhibiting the expression of EGFR and inflammatory response,reducing the damage of neuron cells,and thus achieving analgesia.
