摘要
目的探讨归脾汤加减治疗心脾两虚型孤独症谱系障碍的效果。方法选取160例南阳市第二人民医院收治的孤独症谱系障碍患儿,分为对照组(80例)和观察组(80例),分组方法为随机数字表法,选例时间设置在2020年1月-2023年1月。对照组接受常规康复治疗,观察组在对照组的基础上接受归脾汤加减治疗,两组均治疗3个疗程(4周为1个疗程)。比较两组治疗12周后临床疗效,治疗前、治疗4周后、12周后儿童孤独症评定量表(childhood autism rating scale,CARS)评分、儿童孤独症行为量表(autism behavior checklist,ABC)评分,治疗前及治疗12周后血清白细胞介素-17(interleukin-17,IL-17)、白细胞介素-1β(interleukin-1β,IL-1β)、肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、5-羟色胺(5-hydroxytryptamine,5-HT)、胶质细胞源性神经营养因子(glial cell-derived neurotrophic factor,GDNF)、中枢神经特异蛋白(S100β蛋白)水平及孤独症治疗评估量表(autism treatment evaluation checklist,ATEC)评分。结果治疗12周后,观察组总有效率高于对照组;治疗前~治疗12周后,两组CARS评分、ABC评分逐渐降低,两组间治疗4周、12周后相比,观察组评分更低;治疗12周后与治疗前比较,两组血清IL-17、IL-1β、TNF-α、5-HT、GDNF、S100β蛋白水平,社交、语言、行为、感知评分及总分降低,两组间治疗12周后相比,观察组水平更低,数据存在较显著的差异(P<0.05)。结论归脾汤加减治疗心脾两虚型孤独症谱系障碍患儿有助于缓解其临床症状,抑制体内炎症因子表达,调节神经递质水平,改善行为状况,进而提高临床疗效。
Objective To explore the effect of Modified Guipi Decoction(归脾汤加减)on autism spectrum disorder with deficiency of heart and spleen.Methods A hundred and sixty cases of autism spectrum disorder children admitted to the Second People′s Hospital of Nanyang were divided into the control group(80 cases)and the observation group(80 cases).The grouping method was random number table method,and the selection time was set from January 2020 to January 2023.The control group received routine rehabilitation treatment,and the observation group received Modified Guipi Decoction on the basis of the control group.Both groups were treated for 3 courses(4 weeks was one course).The clinical efficacy after 12 weeks of treatment,childhood autism rating scale(CARS)score and autism behavior checklist(ABC)score before treatment,after 4 and 12 weeks of treatment,the levels of serum interleukin-17(IL-17),interleukin-1β(IL-1β),tumor necrosis factor-α(TNF-α),5-hydroxytryptamine(5-HT),glial cell-derived neurotrophic factor(GDNF),soluble protein-100β(S100β)and autism treatment evaluation checklist(ATEC)before treatment and after 12 weeks of treatment were compared between the two groups.Results After 12 weeks of treatment,the total effective rate of the observation group was higher than that of the control group.Before treatment and after 12 weeks of treatment,the CARS score and ABC score of the two groups decreased gradually,and the score of the observation group was lower than that of the two groups after 4 and 12 weeks of treatment.Compared with those before treatment,after 12 weeks of treatment,the serum levels of IL-17,IL-1β,TNF-α,5-HT,GDNF and S100βprotein,social,language,behavior,perception score and total score of the two groups all decreased,and after 12 weeks of treatment,the levels and the scores of the observation group were lower,with significant differences(P<0.05).Conclusion Modified Guipi Decoction is helpful to relieve the clinical symptoms of autism spectrum disorder children with deficiency of heart and spleen,inhibit the levels of inflammatory factors in the body,regulate the levels of neurotransmitters,improve the behavior and then increase the clinical efficacy.
