摘要
目的 探讨腹腔镜手术联合参芪降糖颗粒在胆结石合并糖尿病患者中的应用效果。方法 选择2023年1月-2024年8月寿光市人民医院收治的94例胆结石合并糖尿病患者为研究对象,根据治疗方法的不同分为两组,各47例。两组均采用常规方法治疗,对照组采用参芪降糖颗粒联合开腹胆囊切除术治疗,观察组采用参芪降糖颗粒联合腹腔镜手术治疗。比较两组血糖水平、免疫应激指标及并发症发生率。结果 干预后,两组空腹血糖、餐后2 h血糖及糖化血红蛋白水平均低于干预前,差异均有统计学意义(P均<0.05);干预后两组血糖水平比较,差异均无统计学意义(P均>0.05)。干预后,两组均出现免疫应激反应,但观察组白细胞介素-6、C反应蛋白、皮质醇及CD8^(+)水平均低于对照组,CD3^(+)、CD4^(+)及CD4^(+)/CD8^(+)均高于对照组,差异均有统计学意义(P均<0.05)。观察组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论 腹腔镜手术联合参芪降糖颗粒能降低胆结石合并糖尿病患者的血糖水平,免疫应激反应相对较轻,且可降低并发症发生率。
Objective To explore the application effect of laparoscopic surgery combined with Shenqi Jiangtang Gran-ules in patients with gallstones and diabetes mellitus.Methods A total of 94 patients with gallstones and diabetes mel-litus admitted to Shouguang People's Hospital from January 2023 to August 2024 were selected as the research ob-jects.They were divided into two groups according to different treatment methods,47 cases in each group.Both groups were treated with conventional methods.The control group was treated with Shenqi Jiangtang Granules combined with open cholecystectomy,and the observation group was treated with Shenqi Jiangtang Granules combined with laparo-scopic surgery.The blood glucose levels,immune stress indexes and incidence of complications were compared be-tween the two groups.Results After intervention,the levels of fasting blood glucose,2-hour postprandial blood glu-cose and glycated hemoglobin A1c in the two groups were lower than those before intervention,and the differences were statistically significant(all P<0.05).There were no significant differences in blood glucose levels between the two groups after intervention(all P>0.05).After intervention,the immune stress response occurred in both groups,but the levels of interleukin-6,C-reactive protein,cortisol and CD8+in the observation group were lower than those in the control group,and the CD3+,CD4+and CD4+/CD8+were higher than those in the control group,the differences were statistically significant(all P<0.05).The incidence of complications in the observation group was lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion Laparoscopic surgery combined with Shenqi Jiangtang Granules can reduce the blood glucose level of patients with gallstones and diabetes mellitus,the immune stress response is relatively light,and the incidence of complications is reduced.
