摘要
目的探讨鼻内针刺联合吡嘧司特钾滴眼液对季节性过敏性结膜炎患者泪液中免疫球蛋白E(IgE)及血清总IgE水平的影响。方法纳入2024年1—6月北京中医药大学东直门医院眼科诊治的季节性过敏性结膜炎患者63例(126只眼),随机分为2组,针药结合组31例(62只眼)给予鼻内针刺双侧内迎香穴联合吡嘧司特钾滴眼液治疗,西药组32例(64只眼)给予氟米龙滴眼液联合吡嘧司特钾滴眼液治疗,2组均连续治疗2周。比较2组患者治疗前、治疗2周后、治疗结束后2周泪液IgE水平、血清总IgE水平、眼部症状评分和眼部体征评分。结果2组患者治疗前、治疗2周后、治疗结束后2周泪液IgE水平、眼部症状评分及眼部体征评分总体比较差异均有统计学意义(P均<0.05);2组患者治疗2周后、治疗结束后2周泪液IgE水平、眼部症状评分及眼部体征评分均明显低于治疗前(P均<0.05),且针药结合组患者治疗结束后2周眼部症状评分明显低于治疗2周后(P<0.05);针药结合组患者治疗2周后和治疗结束后2周泪液IgE水平、眼部症状评分及眼部体征评分均明显低于同期西药组(P均<0.05)。2组患者治疗前后血清总IgE水平组内及组间比较差异均无统计学意义(P均>0.05)。结论鼻内针刺疗法联合吡嘧司特钾滴眼液可明显减轻季节性过敏性结膜炎患者眼部症状体征,并能显著降低泪液IgE水平,但对血清总IgE水平没有明显影响。
Objective It is to explore the effects of intranasal acupuncture combined with pemirolast potassium eye drops on levels of immunoglobulin E(IgE)in tear fluid and total IgE in serum of patients with seasonal allergic conjunctivitis(SAC).Methods Sixty-three SAC patients(126 eyes)treated in the Department of Ophthalmology of Dongzhimen Hospital of Beijing University of Chinese Medicine from January to June 2024 were included and randomly divided into two groups,31 cases(62 eyes)in the needle-medicine combination group were treated with intranasal acupuncture combined with pemirolast potassium eye drops,and 32 cases(64 eyes)in the western medicine group were treated with fluorometholone eye drops combined with pemirolast potassium eye drops,both groups were continuously treated for 2 weeks.The levels of IgE in tear fluid and total IgE in serum,ocular symptom scores and ocular sign scores of patients in the two groups were compared before treatment,after 2 weeks of treatment and in 2 weeks after the end of treatment.Results The differences in tear fluid IgE levels,ocular symptom scores and ocular sign scores were statistically significant in the overall comparison of patients in the two groups before treatment,after 2 weeks of treatment and in 2 weeks after the end of treatment(all P<0.05);the level of tear fluid IgE,ocular symptom scores and ocular sign scores of patients in the two groups after 2 weeks of treatment and in 2 weeks after the end of treatment were significantly lower than those of patients before treatment(all P<0.05),the ocular symptom scores of patients in 2 weeks after the end of treatment were significantly lower than those after 2 weeks of treatment in the acupuncture-medicine combination group(P<0.05);the level of tear fluid IgE,ocular symptom scores and ocular sign scores of patients in the acupuncture-medicine combination group after 2 weeks of treatment and in 2 weeks after the end of treatment were significantly lower than those of patients in the western medicine group in the same period(all P<0.05).There were no statistically significant differences in the level of total IgE in serum between before and after the treatment in each group,and there were no statistically significant differences between the two groups before and after treatment(all P>0.05).Conclusion Intranasal acupuncture combined with pemirolast potassium eye drops can significantly alleviate ocular signs and symptoms and reduce level of IgE in tear fluid in patients with seasonal allergic conjunctivitis,but it has no significant effect on level of total IgE in serum.
