摘要
目的:探讨前馈控制模式在老年日间手术患者术前抗血栓药物管理中的应用效果,为科学管理老年日间手术患者抗血栓药物提供参考。方法:采用便利抽样法,选取首次行日间手术并长期服用抗血栓药物治疗的393例老年患者为研究对象,按照入院时间分为对照组与实验组。对照组采用常规抗血栓药物管理,实验组在对照组基础上进行原因分析,应用前馈控制模式开展药物管理。结果:实验组因抗血栓药当日手术取消率低于对照组,差异具有统计学意义(χ^(2)=6.886,P=0.009);两组药物治疗依从性差异具有统计学意义(χ^(2)=6.658,P=0.037);培训前后,护士在抗血栓药的种类和名称、停用抗血栓药物时间、替代药物名称和用法3个方面的知晓率比较,差异有统计学意义(P<0.05)。结论:前馈控制模式用于术前抗血栓药物管理,可降低当日手术取消率,提高老年日间手术患者药物治疗依从性及护士抗血栓药物管理能力,改善患者就医体验。
Objective:To explore the effects of the preoperative management of antithrombotic drugs based on feedforward control model and to provide a reference for the scientific management of antithrombotic drugs in elderly patients undergoing ambulatory surgery.Methods:By means of convenience sampling,393 elderly patients who received antithrombotic therapy and had their first ambulatory surgery were enrolled.The patients were divided into the control group and the experimental group according to the time of admission.Later the control group was offered conventional management of antithrombotic drugs,while the experimental group received management on the basis of feedforward control model after cause analysis.Results:After the intervention,the cancellation rate of operation caused by antithrombotic drugs in the experimental group was significantly lower than that in the control group(χ^(2)=6.886,P=0.009).Statistically significant differences were observed in the drug treatment adherence between the two groups(χ^(2)=6.658,P=0.037).Moreover,nurses'awareness of the types and names of antithrombotic drugs,the time to discontinue drugs,the names and usage of alternative drugs before and after the training were also statistically different(P<0.05).Conclusion:Preoperative management of antithrombotic drugs based on feedforward control model can reduce the cancellation rate of ambulatory surgery,enhance the medication compliance in elderly patients,optimize nurses'management of antithrombotic drugs to comprehensively improve patients'medical experience.
