摘要
目的:探究磁共振成像在评估膝关节退行性骨关节病患者软骨改变中的价值。方法:回顾柳城县人民医院2020年1月~2023年6月收治的50例膝关节退行性骨关节病患者资料,所有患者均接受磁共振成像PDWI-FS-TSE、T1WI-TSE检查,以关节镜为金标准,探究各序列诊断软骨损伤的效能及判断损伤分级的准确率。结果:50例患者检查软骨234处,包括正常软骨145处,损伤软骨89处,损伤分级包括:Ⅰ级11处,Ⅱ级21处,Ⅲ级22处,Ⅳ级35处。磁共振成像诊断软骨损伤,特异度:联合诊断>PDWI-FS-TSE>T1WI-TSE,灵敏度:联合诊断=T1WI-TSET1WI-TSE。结论:磁共振成像在诊断膝关节退行性骨关节病患者软骨损伤中与关节镜诊断结果具有良好一致性,存在较高损伤分级诊断准确率和特异度。
Objective:To explore the value of magnetic resonance imaging in evaluating cartilage changes in patients with knee degenerative osteoarthritis.Methods:50 patients with degenerative osteoarthritis of the knee joint admitted to Liucheng County People's Hospital from January 2020 to June 2023 were selected as the study subjects.All patients underwent magnetic resonance imaging examination,using PDWI-FS-TSE and T1WI-TSE sequences as the gold standard,and arthroscopy as the gold standard.The efficacy of each sequence in diagnosing cartilage injury and the accuracy of judging injury grading were explored.Results:Among 50 patients,234 cartilage locations were examined,including 145 normal cartilage locations and 89 damaged cartilage locations.The injury grades included 11 grade I cartilage locations,21 grade II cartilage locations,22 grade III cartilage locations,and 35 grade IV cartilage locations.Magnetic resonance imaging diagnosis of cartilage injury specificity results:joint diagnosis>PDWI-FS-TSE>T1WI-TSE,diagnostic sensitivity.Joint diagnosis=T1WITSE<PDWI-FS-TSE,accuracy of cartilage injury grading results:joint diagnosis=PDWI-FS-TSE>T1WI-TSE.Conclusion:Magnetic resonance imaging has good consistency with arthroscopic diagnosis in the diagnosis of cartilage injury in patients with knee degenerative osteoarthritis,with high accuracy and specificity in injury grading diagnosis.
