摘要
目的:系统评价中药内服治疗变应性接触性皮炎(ACD)的临床疗效及安全性,并发掘选方用药规律。方法:计算机检索中国知网(CNKI)、万方(WANFANG,WF)、维普(VIP)、中国生物医学文献(CBM)、PubMed、Embase、Web of Science。纳入检索时限为1999年1月—2024年2月已发表的关于中药内服治疗ACD的随机对照试验,根据纳入和排除标准,采用NoteExpress软件对文献进行筛选,RevMan 5.4.1软件分析数据。结果:共纳入12项研究,涉及病例1013例,其中试验组511例,对照组502例。Meta分析结果显示:中药内服治疗ACD的结局指标:总有效率[RR=1.32,95%CI(1.23,1.42),Z=7.37(P<0.000 01)],痊愈率[RR=1.53,95%CI(1.33,1.76),Z=5.98(P<0.000 01)],VAS评分[MD=-1.13,95%CI(-1.37,-0.89),Z=9.35(P<0.000 01)],血清Th1/Th2水平[MD=1.02,95%CI(0.54,1.51),Z=4.16(P<0.0001)],EBV病毒抗体水平[MD=-0.72,95%CI(-0.83,-0.61),Z=13.07(P<0.000 01)],不良反应评价[RR=0.52,95%CI(0.29,0.91),Z=2.29(P<0.02)],均优于西药对照组,差异有统计学意义。剔除相同方剂,共纳入6首方剂。涉及28味中药,总使用频次76次;生地黄、防风、丹皮、金银花等为高频使用药物,功效以清热凉血、清热解毒、清热燥湿为主。结论:中药内服治疗ACD疗效显著,安全性较好,较西药治疗具有一定优势。
Objective:To systematically evaluate the clinical efficacy and safety of oral administration of Chinese medicine in the treatment of allergic contact dermatitis(ACD)and explore the rules of prescription selection and medication.Methods:CNKI,WANFANG,VIP,CBM,Pubmed,Embase and Web of Science were searched by computer.Randomized controlled trials on the treatment of ACD with oral administration of Chinese medicine were included with a retrieval time limit of January 1999 to February 2024.According to the inclusion and exclusion criteria,NoteExpress software was used to screen the literature.RevMan5.4.1 software was used to analyze the data.Results:A total of 12 studies involving 1013 cases were included.There were 511 cases in the experimental group and 502 cases in the control group..The results of meta-analysis showed that the outcome indicators of ACD treated with oral administration of Chinese medicine were total effective rate[RR=1.32,95%CI(1.23,1.42),Z=7.37(P<0.00001)],cure rate[RR=1.53,95%CI(1.33,1.76),Z=5.98(P<0.00001)],VAS score[MD=-1.13,P<0.05].95%CI(I-1.37,-0.89),Z=9.35(P<0.00001)],serum Th1/Th2 level[MD=1.02,95%CI(0.54,1.51),Z=4.16(P<0.0001)],EBV virus antibody level[MD=-0.72,95%C(I-0.83,-0.61),Z=13.07(P<0.00001)],and adverse reaction evaluation[RR=0.52,95%C(I 0.29,0.91),Z=2.29(P<0.02)],which were better than those in the control group with western medicine,and the differences were statistically significant.After excluding the same prescription,a total of 6 prescriptions were included..It involved 28 kinds of Chinese medicines,and the total frequency of use was 76 times.Shengdihuang,Fangfeng,Danpi and Jinyinhua were high-frequency drugs,and their efficacy was mainly for clearing heat and cooling blood,clearing heat and toxic materials,and clearing heat and drying dampness.Conclusion:Oral administration of Chinese medicine is effective and safe in the treatment of ACD,which has certain advantages over western medicine.
