摘要
脉诊是中医诊疗疾病时唯一通过接触人体获得生命信息的方法,是辨病与辨证的判定指标,对于提高临床辨证和指导针灸施术具有重要作用。然早期经典医理精深,所载脉法不一,至今仍未有统一的切脉辨证标准与审脉针灸诊疗体系。该文结合《黄帝内经》等中医经典中脉学与针灸理论及现代临床研究,发现早期脉诊多用于指导经脉辨证和针灸选穴,其记载脉法有三:三部九候法诊遍身之脉以辨经络、人迎寸口法诊阴阳之脉以辨盛衰、独取寸口法诊脏腑之脉以辨虚实。通过不同脉法审察脏腑阴阳气血盛衰、辨别表里寒热虚实,指导针灸补泻、针具选择、针刺深度,检验临床疗效及提示针灸禁忌,使临床诊疗便捷而效专,对临床审脉辨证施针有重要指导作用。
Pulse diagnosis is the only method for TCM to obtain vital information by contacting the human body when diagnosing and treating diseases.It is an indicator for distinguishing diseases and syndromes,and plays an important role in improving clinical syndrome differentiation and guiding acupuncture.However,the early classic medical theory is profound,and the pulse methods recorded are different.There is still no unified pulse diagnosis standard and pulse examination acupuncture diagnosis and treatment system.This paper combines the pulse science and acupuncture theory in TCM classics such as“Huangdi Neijing”and modern clinical research,and finds that early pulse diagnosis is mostly used to guide meridian syndrome differentiation and acupuncture point selection.There are three pulse methods recorded:the three-part nine-hour method to diagnose the pulse of the whole body to distinguish the meridians,the Renying Cunkou method to diagnose the Yin and Yang pulses to distinguish prosperity and decline,and the single Cunkou method to diagnose the pulse of the internal organs to distinguish deficiency and excess.Through different pulse methods,the rise and fall of Yin and Yang,Qi and blood in the internal organs can be examined,and the cold and heat,deficiency and excess of the exterior and interior can be distinguished.Acupuncture tonification and drainage,needle selection,and acupuncture depth can be guided.Clinical efficacy can be tested and acupuncture taboos can be prompted.This makes clinical diagnosis and treatment convenient and effective,and plays an important guiding role in clinical pulse examination and syndrome differentiation and acupuncture.
