摘要
目的探讨C-PLAN指数、OPS评分与重症肺炎患者的相关临床特征及生存预后的关系,构建生存预测模型并进行效能评价。方法回顾性分析了2021年1月1日至2024年3月31日于内蒙古自治区人民医院诊断为重症肺炎的213例患者的临床资料;采用Logistic分析影响重症肺炎患者的预后因素;使用R version4.4.0构建预后列线图。结果共计213例患者,住院28天内死亡率33.8%;C-PLAN指数>2分、OPS评分≥2分、APACHEⅡ评分>18分组的患者预后更差;Logistic分析发现:休克、APACHEⅡ评分、C-PLAN指数和OPS评分均为影响预后的危险因素(P<0.05);据此构建的预后列线图具有较好的预测效能。结论C-PLAN指数≤2分、OPS评分<2分的患者有更好的预后,C-PLAN指数和OPS评分有望成为预测重症肺炎患者预后的潜在指标;休克、APACHEⅡ评分、C-PLAN指数和OPS评分均为影响预后的危险因素,以此构建重症肺炎患者预后的列线图模型能对患者的生存做出较好预测。
Objective To explore the relationship of C-PLAN index and OPS score with relevant clinical features and survival prognosis of patients with severe pneumonia,and to construct survival prediction model and evaluate its efficacy.Methods The clinical data of 213 patients diagnosed with severe pneumonia in Inner Mongolia Autonomous Region People's Hospital from January 1,2021 to March 31,2024 were retrospectively analyzed.The prognostic factors of patients with severe pneumonia were analyzed by Logistic analysis.The prognosis nomogram was constructed using R version 4.4.0.Results There were a total of 213 patients,and the mortality within 28 days of hospitalization was 33.8%.Those patients with C-PLAN index>2,OPS score≥2 and APACHEⅡscore>18 had worse prognosis.Logistic analysis showed that shock,APACHEⅡscore,C-PLAN index and OPS score were all risk factors affecting prognosis(P<0.05).The prognosis nomogram constructed by this method has good predictive efficiency.Conclusion Those patients with C-PLAN index≤2 and OPS score<2 have better prognosis.C-PLAN index and OPS score are expected to be potential prognostic indicators for patients with severe pneumonia.Shock,APACHEⅡscore,C-PLAN index and OPS score are all risk factors affecting prognosis.The prognosis of patients with severe pneumonia can be better predicted by constructing a nomogram model.
