摘要
目的采用16S rRNA测序技术对细菌性肺炎患者和非细菌性肺炎患者的肺泡灌洗液微生物菌群的变化进行分析。方法选择2022年6月到2023年1月在泉州市第一医院呼吸与危重症医学科住院的细菌性肺炎患者20例作为研究对象,以同期非细菌性肺炎患者20例作为对照组。收集入选患者的临床资料、肺泡灌洗液。通过16S rRNA测序检测肺泡关系菌群,并进行生物学信息分析。结果肺炎组的16S rRNA测序得到OTU总数为5857,对照组的OTU为15379,Alpha多样性结果显示在肺炎组的微生物种群丰度等级降低且差异具有统计学意义,Beta多样性分析发现肺炎组和对照组样本间的差异显著。分析肺泡灌洗液的种群组成差异发现,在门、科水平20种群中发现肺炎丰度与对照组相比,益生菌丰度降低而促炎病原菌的丰度升高且差异具有统计学意义。结论细菌性肺炎患者肺泡灌洗液的微生态失衡表现为益生菌丰度降低、致病菌菌群的群落结构发生显著改变,这可能与细菌性肺炎的发生发展有关。
Objective To analyze the changes of microbial flora in alveolar lavage fluid of patients with bacterial pneumonia and volunteers with non-bacterial pneumonia by 16S rRNA sequencing technology.Methods From June 2022 to January 2023,20 patients hospitalized in the Department of Respiratory and Critical Care Medicine of the First Hospital of Quanzhou with bacterial pneumonia,and 20 patients with non-bacterial pneumonia were selected as the control group.Their clinical data and alveolar lavage fluid were collected.The alveolar related flora was detected by 16S rRNA sequencing,and the biological information was analyzed.Results The total number of OTU obtained by 16S rRNA sequencing in the pneumonia group was 5857,and the OTU in the control group was 15379.The alpha diversity results showed that the microbial population abundance grade in the pneumonia group decreased and the difference was statistically significant.The beta diversity analysis showed that there was a significant difference between the samples in the pneumonia group and the control group.The analysis of population composition differences of alveolar lavage fluid showed that compared with the control group,the abundance of probiotics decreased and the abundance of proinflammatory pathogens increased in 20 populations at phyla and family level,and the difference was statistically significant.Conclusion The microecological imbalance of alveolar lavage fluid in patients with bacterial pneumonia shows that the abundance of probiotics decreases and the community structure of pathogenic bacteria changes significantly,which may be related to the occurrence and development of bacterial pneumonia.
