摘要
目的:探讨急诊气管插管病人口唇黏膜压力性损伤的发生现状及影响因素,以期为临床预防提供依据。方法:采用便利抽样法,选取2022年6月—2023年10月北京某三级甲等医院急诊科收治的166例气管插管病人,收集病人的一般资料及相关因素,分析气管插管病人口唇黏膜压力性损伤发生现状及影响因素。结果:共32例(19.3%)病人发生口唇黏膜压力性损伤;Logistic回归分析显示,插管型号、插管留置天数、口腔分泌物情况及躁动程度为气管插管病人口唇黏膜压力性损伤发生的影响因素。结论:气管插管病人口唇黏膜压力性损伤的发生部位主要集中在上唇和下唇,以破溃为主。插管型号越大、插管留置时间越长、口腔分泌物越多及病人越躁动其发生风险越高。
Objective:To explore the current status and influencing factors of pressure injuries on the lip mucosa among emergency intubated patients,aiming to provide a basis for clinical prevention.Methods:Using convenience sampling,166 intubated patients admitted to the emergency department of a tertiary hospital in Beijing from June 2022 to October 2023 were selected.General information and relevant factors of the patients were collected to investigate the influencing factors of pressure injuries on the lip mucosa among intubated patients.Results:A total of 32 patients(19.3%)developed pressure injuries on the lip mucosa;Logistic regression analysis showed that endotracheal tube size,duration of intubation,oral secretion status,and agitation level were influencing factors for the occurrence of pressure injuries on the lip mucosa among intubated patients.Conclusion:Pressure injuries on the lip mucosa among intubated patients mainly occurred on the upper and lower lips,with ulceration being the primary manifestation.Larger endotracheal tube size,longer duration of intubation,more oral secretions,and patient agitation can increase the risk of these injuries.
